Improvement of disease management ability and glycaemic control in patients with T2DM by continuity of care interventions based on context of universal health belief model
Objective To understand the impact of a continuity of care intervention on patients with type 2 diabetes mellitus(T2DM)in the context of a universal health belief model.Methods 120 cases of T2DM patients admitted to the hospital from January 2023 to January 2024 were chosen and divided into two groups,60 in each,according to the random double-blind method.The control group used conventional continuity of care intervention,and the observation group additionally used continuity of care intervention based on the context of the universal health belief model.The two groups were compared in terms of disease management ability,glycaemic control level,re-hospitalisation rate and quality of life.Results Post-intervention medication adherence score(7.28±0.36)in the observation group was higher than that of the control group(6.21±0.40),self-management ability score(82.43±4.13)was higher than that of the control group(75.18±4.20),and blood glucose level was significantly lower compared with the control group(P<0.05).The post-intervention diabetes to physical,social relationship,psychological/spiritual and therapeutic dimensions scores of the observation group were lower than those of the control group(P<0.05).No diabetic foot,diabetic nephropathy,diabetic ketoacidosis,or readmission occurred in both groups after 6 months of follow-up.Conclusion The continuity of care intervention based on the context of the universal health belief model can improve the self-management ability of T2DM patients,and the effect of glycaemic control is good and is worth applying.
