益生菌联合经鼻空肠管营养支持对头颈部肿瘤患者同步放化疗期间营养状况的影响

目的 探讨益生菌联合经鼻空肠管营养支持对头颈部肿瘤患者同步放化疗期间营养状况的影响。方法 选取2020 年 7 月至 2022 年 7 月在南通市肿瘤医院进行同步放化疗的 80 例头颈部肿瘤患者作为研究对象,按照随机数字表法将其分为观察组(益生菌联合经鼻空肠管营养支持,40 例)与对照组(经鼻空肠管营养支持,40 例)。比较 2 组患者治疗效果、不良反应及干预后欧洲癌症研究治疗组织癌症患者生活质量评价量表(EORTC QLQ-C30)评分,比较 2 组患者干预前后的T淋巴细胞亚群(CD3+、CD4+、CD4+/CD8+)及血清白蛋白(ALB)、前白蛋白(PA)、血红蛋白(HB)水平。结果 2 组患者客观缓解率比较差异无统计学意义(P>0。05)。干预后 2 组患者CD3+、CD4+、CD4+/CD8+高于干预前,且观察组高于对照组(P<0。05);干预后观察组ALB、PA、HB水平与干预前比较差异无统计学意义(P>0。05),对照组ALB、PA、HB水平低于干预前(P<0。05);干预后观察组ALB、PA、HB水平高于对照组(P<0。05)。观察组不良反应总发生率低于对照组(P<0。05)。干预后观察组EORTC QLQ-C30 各项评分优于对照组(P<0。05)。结论 益生菌联合经鼻空肠管营养支持用于头颈部肿瘤患者同步放化疗可以更好地改善营养状况。
Effect of probiotics combined with nasojejunal nutritional support on nutritional status of patients with head and neck cancers during concurrent chemoradiotherapy
Objective To investigate the effect of probiotics combined with nasojejunal nutritional support on nutritional status of patients with head and neck cancers during concurrent chemoradiotherapy.Methods A total of 80 patients with head and neck tumors who received concurrent chemoradiotherapy in Nantong Tumor Hospital from July 2020 to July 2022 were selected as research objects.They were divided into observation group(probiotics combined with nasojejunal nutritional support,n=40)and control group(nasojejunal nutritional support,n=40)according to the random number table method.The treatment effect,side effects and the score of European Organization for Research and Treatment of Cancer quality of life questionnaire(EORTC QLQ-C30)after intervention were compared between the two groups.The levels of T lymphocyte subsets(CD3+,CD4+,CD4+/CD8+),serum albumin(ALB),prealbumin(PA)and hemoglobin(HB)before and after intervention were compared between the two groups.Results There was no significant difference in objective response rate between the two groups(P>0.05).CD3+,CD4+and CD4+/CD8+after intervention were significantly higher than those before intervention in both groups,and these indexes in the observation group were significantly higher than those in the control group after intervention(all P<0.05).There were no significant differences in ALB,PA or HB in the observation group before and after intervention(all P>0.05).The ALB,PA and HB in the control group after intervention were significantly lower than those before intervention,and these indexes in the observation group were significantly higher than those in the control group after intervention(all P<0.05).The total incidence of side effects in the observation group was significantly lower than that in the control group(P<0.05).After intervention,the scores of EORTC QLQ-C30 in the observation group were superior to those in the control group(all P<0.05).Conclusion Probiotics combined with nasojejunal nutritional support can improve the nutritional status of patients with head and neck cancers during concurrent chemoradiotherapy.
