The study observes and analyzes the effect of hospital-community-family linkage nursing intervention in the prevention and treatment of hypertension.The improvement of the quality of life of hypertensive patients is particularly important.The study selects 120 patients with hypertension in the hospital and divides them into control group and observation group,with 60 cases in each group.Control group receive routine nursing,and observation group receive hospital-community-family linage nursing intervention.All evaluation index of two groups are compared.The results show that SBP and DBP of observation group are significantly lower than control group(P<0.05).Self-management ability and quality of life of observation group are significantly better than control group(P<0.05).The application of hospital-community-family linkage nursing intervention in the prevention and treatment of hypertension can better control the blood pressure level of hypertension patients,and improve their self-management ability and quality of life.
