DRG支付改革对A市中医院运行的影响

分析A市某三甲中医院在DRG支付改革下面临的问题,提出相应对策.采用间断时间序列模型分别对改革前(2019 年1-12 月)与改革后(2020 年1 月—2021 年12 月)共三年的3 个医疗服务效率指标、4 个费用控制指标、6 个中医医疗服务水平指标共13 个指标数据进行分析.结果显示,改革后平均住院日呈下降趋势,次均门诊费用增速放缓.材料费用占比呈上升趋势.中药饮片收入占药品收入比例、住院中医医疗服务项目收入占住院医疗收入比例、中医药治疗收入占住院医疗收入比例、出院患者使用中医非药物疗法的比例呈下降趋势,出院患者中药饮片使用率增速放缓.基于此,建议针对不同治疗特点的病种采取不同的医保支付方式,继续加大医保对中药的政策支持力度,加强费用控制,适时调整付费标准以体现中医"简便验廉"的优势.
Abstract
The study analyzes the problems faced by a tertiary Chinese medicine hospital in A City under the DRG payment reform,and puts forward corresponding countermeasures.The study analyzes the data of a total of 13 indicators,including 3 indicators of medical service efficiency,4 indicators of cost control and 6 indicators of TCM medical service level in three years before the reform(from January to December,2019)and after the reform(from January to December,2021)with discontinuous time series model.The results show that after the reform,the average days of hospitalization show a downward trend,and the growth rate of average outpatient expenses slows down.The proportion of material cost rises.The proportion of TCM decoction pieces income in drug income,the proportion of inpatient TCM medical service income in inpatient medical income,the proportion of TCM treatment income in inpatient medical income,the proportion of discharged patients with TCM non-drug therapy show decreasing trend.The growth rate of the utilization rate of TCM decoction pieces in discharged patients slows down.Based on this,it is suggested to adopt different medical insurance payment methods for diseases with different treatment characteristics,continue to increase the policy support of medical insurance for traditional Chinese medicine,strengthen the cost control,and adjust the payment standard timely to reflect the advantage of"simple incorrupt inspection"of TCM.
