Evaluation of the Application of Platelet-rich Fibrin(PRF)in Minimally Invasive Percutaneous Repair of Acute Achilles Tendon Rupture

摘要
评价富血小板纤维蛋白(Platelet-rich fibrin,PRF)用于急性跟腱断裂修复手术的应用效果,为患者术后的早日康复提供理论依据.选取34 例急性闭合性跟腱断裂且行经皮微创修复手术的患者为研究对象,随机分为对照组和实验组,每组各 17 例.对照组采用经皮微创修复手术,实验组在对照组的基础上注射PRF,比较两组患者受伤至手术时间、VAS评分、AOFAS评分情况、术中相关指标、跟腱活动能力、术后视觉模拟评分法(Visual analogue scales,VAS)评分、术后并发症发生率及术后跖屈肌力.结果表明,实验组患者的住院时间及伤口愈合时间显著短于对照组(P<0.05);实验组术后1 个月、3 个月跟腱完全断裂评分(achilstcendon total rupture score,ATRS)及美国足与踝关节协会(American Orthopedic Foot and Ankle Society,AOFAS)的评分均显著高于对照组(P<0.05);实验组术后1、3 个月VAS评分显著低于对照组(P<0.05);两组患者的术后并发症发生率差异不显著(P>0.05);实验组患者术后3 个月时跖屈肌力显著高于对照组(P<0.05).说明PRF应用于急性闭合性跟腱断裂后经皮微创修复手术患者可以加快患者跟腱的愈合及康复速度,改善其跟腱功能状态,并缩短患者的疼痛病程.
Abstract
The study evaluates the application effect of Platelet-rich fibrin(PRF)in the repair of acute Achilles tendon rupture,so as to provide theoretical basis for the early recovery of patients after the operation.The study selects 34 patients with acute closed Achilles tendon rupture who underwent percutaneous minimally invasive repair surgery as the study objects,and randomly divides them into control group and experimental group,with 17 cases in each group.The control group receive percutaneous minimally invasive repair surgery,and the experimental group receive PRF injection on the basis of the control group.Then the study compares the time from injury to operation,VAS score,AOFAS score,relevant indexes during operation,Achilles tendon mobility,Visual analogue scales(VAS)score,incidence of postoperative complications and postoperative plantar flexion muscle strength between the two groups.The results show that the hospital stay and wound healing time of the experimental group were significantly shorter than those of the control group(P<0.05).The achilstcendon total rupture score(ATRS)and the American Orthopedic Foot and Ankle Society(AOFAS)1 and 3 months after surgery were significantly higher than those of control group(P<0.05).VAS scores of the experimental group were significantly lower than those of the control group 1 and 3 months after operation(P<0.05).There was no significant difference in the incidence of postoperative complications between two groups(P>0.05).The metatarsal flexion strength of the experimental group was significantly higher than that of the control group 3 months after operation(P<0.05).This indicates that the application of PRF in patients with acute closed Achilles tendon rupture can accelerate the healing and rehabilitation of the Achilles tendon,improve the functional status of the Achilles tendon,and shorten the pain course of the patients.
