Correlation of L5 Isthmic Cleft Slip Fraction with Lumbar Spine Parameters in the Sagittal Planes of DR and MRI
Objective:DR and MRI sagittal parameters, degree of disc degeneration at L5/S1 and adjacent segments above, and Modic changes were analyzed in patients with L5 isthmic cleft slip, and the correlation between lumbar spine sagittal parameters and slip grading was investigated.Methods:106 patients with degree Ⅰ and Ⅱ L5 isthmus cleft slip treated in the hospital from September 2016 to August 2022 with complete data were selected for the study.Slippage Ⅰ degree and Ⅱ degree slippage were di-vided into two groups, and the differences between the basic information and relevant lumbar spine parameters were compared be-tween the two groups of patients. The patients'lumbar lordosis angle(LL),slip fraction, lumbosacral angle(LSA),and sacral tilt angle(SS)were measured. The degree of disc degeneration and disc herniation at L4/5 and L5/S1 were assessed in the sagittal plane of lumbar DR and MRI using the Pfirrmann grading criteria, and the endplate Modic condition was evaluated. Results:There was a statistically significant difference in gender, age,Modic changes in the slipped segment,LL,and Pfirrman grading of the adja-cent segment above the slipped segment(χ2=6.315,t=3.352,χ2=7.945,0.200;t=2.641;χ2=4.355,8.449;P<0.05).Conclu-sion:In L5 isthmic cleft slip, the slip was more severe in women and older patients, and the incidence of Modic changes in the slipped segments was higher in patients with severe slip,so the anterior lumbar kyphosis should be emphasized in clinical fol-low-up and surgery.
