Objective:To analyze the diagnostic value of magnetic resonance imaging(MRI)for pelvic lateral lymph node me-tastasis(PLLNM)in rectal cancer.Methods:80 cases of rectal cancer patients admitted to the hospital from March 2020 to February 2022 were selected as the study subjects,and all the patients received MRI examination and pathological biopsy to confirm the diagnosis before operation,and all of them underwent rectal cancer resection and pelvic lymph node dissection.The accuracy,sen-sitivity,specificity and consistency of MRI for PLLNM of rectal cancer were calculated using pathological findings as the gold stan-dard,and MRI image features of lymph nodes of different natures were compared.Pearson test was used to analyze the correlation between MRI image features and PLLNM of rectal cancer,and a multifactorial logistic regression analysis model was used to ana-lyze the independent risk factors affecting PLLNM.Results:A total of 29 cases of PLLNM and 51 cases of non-metastasis were de-tected in 80 patients with rectal cancer diagnosed by pathology,a total of 35 cases of PLLNM and 45 cases of non-metastasis were detected by MRI diagnosis,and the diagnostic concordance between the two was good(Kappa=0.845).The percentage of blurred margins and model inhomogeneity was significantly higher in patients with PLLNM than in those without metastasis,and the short diameter of lymph nodes was significantly longer than that of those without metastasis,with a statistically significant difference(χ2=12.342,8.136;t=13.857,P<0.01).Pearson correlation analysis showed that the detection rate of rectal cancer PLLNM had a positive correlation with lymph node short diameter,blurred margins and signal inhomogeneity(r>0,P<0.001).The multifactorial logistic regression analysis model showed that lymph node signal was an independent risk factor for PLLNM in rectal cancer.Con-clusion:The diagnostic efficacy of MRI for rectal cancer PLLNM is high,and there are obvious differences in the image characteristics of lymph nodes with different properties,among which the signal of lymph nodes,fuzzy edges and uneven signals are closely related to PLLNM,and the signal of lymph nodes is an independent risk factor for rectal cancer PLLNM.The measurement of lymph node short diameter by MRI is a reliable parameter for diagnosing PLLNM in rectal cancer and has a positive effect on the early prediction of PLLNM.
