Study on the Applied Value of Perineal Pelvic Floor Ultrasonography in Assessing the Relationship Between Different Modes of Delivery and Maternal Pelvic Floor Function
Objective:To investigate the value of perineal pelvic floor ultrasonography in assessing the relationship between different modes of delivery and maternal pelvic floor function.Methods:160 cases of primigravid women who gave birth in the hospital from January 2020 to April 2022 were selected as the study subjects,which were divided into cesarean section group and vaginal delivery group according to the different delivery methods,with 80 cases in each group.80 cases of healthy infertile women who underwent health checkups at the same period of time were also selected as the control group.Perineal pelvic floor ultrasonography was performed by GE Voluson S8 color Doppler ultrasound diagnostic instrument at 6~8 weeks postpartum in both groups,and perineal pelvic floor ultrasonography was performed during physical examination in healthy women.The ultrasound parameters of pelvic floor function in resting and Valsalva states were observed and measured and the differences in pelvic floor ultrasound parameters among the three groups of women in different states were compared.Results:At resting state,the angle of inclination of the urethra was smaller in the cesarean section group and the vaginal delivery group than in the control group,the angle of inclination of the urethra was smaller in the vaginal delivery group than in the cesarean section group,and the difference was statistically significant(F=196.971,P<0.05).The differences in bladder neck position,bladder position,external cervical opening position,bladder neck mobility,urethral inclination angle,urethral rotation angle,posterior vesicourethral angle,and cleavage area were statistically significant among the three groups of females in the Valsalva state(F=888.883,803.493,391.602,223.600,100.175,172.614,165.113,41.806,P<0.05).The incidence of bladder prolapse and urinary leakage was higher in the vaginal cesarean group than in the cesarean group,and the difference was statistically significant(χ2=17.604,7.910,P<0.05).Conclusion:Delivery affects the structure and function of the pelvic floor.Transperineal pelvic floor ultrasonography can clearly show the structure of the pelvic floor after delivery,in which the abnormalities of ultrasound parameters of the pelvic floor structure are more obvious in the case of vaginal delivery compared with cesarean section,which is closely related to the damage of the pelvic floor structure and function,and it can provide reference for the diagnosis of the pelvic floor dysfunction after delivery and the rehabilitation treatment.
