Analysis of the Diagnostic Value of Ultrasonography Scores in Simple Acute Appendicitis and Acute Suppurative Appendicitis
Objective:To analyze the diagnostic value of ultrasonography scores in simple acute appendicitis and acute suppurative appendicitis.Methods:80 patients with acute appendicitis who underwent appendiceal ultrasonography and clinically confirmed diagnosis admitted to the hospital from January 2019 to January 2020 were selected as the study group,and 80 patients with non-acute appendicitis admitted during the same period were selected as the control group.All patients were scored according to the appendiceal ultrasonography scoring system.The scores of ultrasonography scores were compared between acute appendicitis and non-acute appendicitis,and between simple acute appendicitis and acute septic appendicitis.Subjects'work characteristics(ROC)curves were plotted and area under the curve(AUC)was calculated.Results:The ultrasonography score of the study group was significantly higher than that of the control group,and the difference was statistically significant(t=10.174,P＜0.05).The ROC curve showed that the cut-off value of the ultrasonography score for the diagnosis of acute appendicitis versus non-acute appendicitis was 4,with an AUC of 0.869(95%CI 0.806～0.917),a maximal Yoden's index of 0.625,a sensitivity of 81.25%,and a specificity of 81.25%.The ultrasonography score was significantly higher in the acute suppurative appendicitis group than in the simple acute appendicitis group,and the difference was statistically significant(t=4.086,P＜0.05).The ROC curve showed a cutoff value of 4 for the ultrasonography score to diagnose acute versus non-acute appendicitis,with an AUC of 0.754(95%CI:0.645～0.843),a maximum Yoden's index of 0.403,a sensitivity of 97.87%,and a specificity of 42.42%.Conclusion:The use of ultrasonography scoring has good diagnostic value for diagnosing acute versus non-acute appendicitis,and simple versus acute suppurative appendicitis.
