Analysis of the Effect of Different Lengths of PFNA Intramedullary Nails on Bone Metabolic Profile and Postopera-tive Stress in Patients with Intertrochanteric Femoral Fractures
Objective:To observe the effect of different lengths of anti-rotation proximal femoral(PFNA)intramedullary nails in intertrochanteric femur fracture surgeries and their effects on patients'bone metabolism and postoperative stress.Methods:76 patients with intertrochanteric fracture of the femur who underwent PFNA surgical treatment in the hospital from January 2021 to May 2022 were selected as the study subjects,and they were equally divided into two groups by lottery method.There were 38 patients in group A,and the intramedullary nails with specifications of 170 mm were applied during the operation,and there were 38 patients in group B,and the intramedullary nails with specifications of 240 mm were applied during the operation.The surgical condition,fracture healing,postoperative stress index hip function recovery and postoperative complications were compared between the two groups.Results:The time-consuming surgery and the number of intraoperative fluoroscopy in group A were less than that in group B,but the fracture healing time was higher than that in group B,with statistically significant differences(t=2.428,3.308;P<0.05).Bone mineral density,osteoprotective pigment(OPG),and bone-specific alkaline phosphatase(BALP)in group B were higher than that in group A,with statistically significant differences(t=2.171,2.610,2.141;P<0.05).The postoperative hip function Harris score and Mayo score of group B were higher than that of group A,and the difference was statistically significant(t=2.214,2.545;P<0.05).Conclusion:When different lengths of intramedullary nails were used to perform PFNA surgery on patients with intertrochanteric femoral fractures,the intraoperative bleeding,drainage volume,postoperative stress,and the risk of postoperative complications were all lower,and the long intramedullary nails had a higher number of intraoperative fluoroscopies and took a longer time for the surgery,but the long intramedullary nails were more conducive to the improvement of bone metabolism level,the promotion of the fracture healing,and the restoration of the hip function of the patients,compared with the short intramedullary nails.
