摘要

目的:探讨"医养结合"四元联动下的家庭养疗分级护理在老年慢性阻塞性肺疾病(COPD)患者院外护理中的应用效果.方法:选取2021年6月—2022年5月厦门弘爱医院收治的112例老年COPD患者作为研究对象,将其分为对照组和研究组,每组各56例.对照组患者给予常规护理干预,研究组患者给予"医养结合"四元联动下的家庭养疗分级护理干预.比较两组患者护理前后心理状态、睡眠质量、生活质量及护理质量情况.结果:护理后,研究组患者抑郁自评量表(SDS)、焦虑自评量表(SAS)评分低于对照组,差异有统计学意义(t=7.336、7.925,P＜0.05).研究组患者护理1周后、4周后、8周后匹兹堡睡眠质量指数评估量表(PSQI)评分低于对照组,差异有统计学意义(t=9.336、10.235、12.033,P＜0.05).护理后,研究组患者社会功能、躯体功能、角色功能、认知功能评分高于对照组,差异有统计学意义(t=6.532、7.265、5.433、6.336,P＜0.05).护理后,研究组患者业务水平、护理态度、心理干预、健康宣教优于对照组,差异有统计学意义(t=6.363、5.435、4.366、5.036,P＜0.05).结论:对老年COPD患者行"医养结合"四元联动下的家庭养疗分级护理,能够改善患者心理状态,提高其睡眠质量、生活质量,护理效果较好.

Abstract

Objective:To explore the effect of family nursing grading nursing under the four linkages of"combination of medical care and nursing care"out of hospital nursing of elderly COPD.Methods:112 elderly patients with COPD who received inpatient treatment and nursing in the hospital from June 2021 to May 2022 were included.They were divided into 56 cases in the control group and 56 cases in the study group.Among them,the elderly patients with COPD in the control group were given routine nursing measures.The elderly patients with COPD in the study group were treated with family nursing and graded nursing under the four linkages of"combination of medical care and nursing care".After nursing,the mental state level,sleep quality level,quality of life score and nursing quality of the two groups were compared and analyzed respectively.Results:After nursing,the SDS and SAS scores of the study group were lower than those of the control group,and the difference was statistically significant(t=7.336,7.925;P＜0.05).PSQI scores of the study group after 1 week,4 weeks and 8 weeks were lower than those of the control group,and the difference was statistically significant(t=9.336,10.235,12.033;P＜0.05).After nursing,the scores of social function,physical function,role function and cognitive function in the study group were higher than those in the control group,and the differences were statistically significant(t=6.532,7.265,5.433,6.336;P＜0.05).After nursing,the patients'professional level,nursing attitude,psychological intervention and health education in the study group were better than those in the control group,and the differences were statistically significant(t=6.363,5.435,4.366,5.036;P＜0.05).Conclusion:The overall nursing effect of the elderly patients with COPD treated by family nursing grading nursing under the"combination of medical care and nursing care"quaternion linkage is good.
