Distribution of Pathogenic Bacteria and Prognostic Risk Factors in Patients with Severe Pneumonia
Objective:To analyze the distribution of pathogenic bacteria in patients with severe pneumonia and its prognostic risk factors.In order to provide a reliable basisi for clinic.Methods:The medical records of 90 patients with severe pneumonia admitted to the hospital from April,2020 to April,2022 were analyzed retrospectively.And the clistribution of pathogenic bacteria was andyzed In addition,according to their clinical outcomes(death or survival),they were divided into good prognosis group(n=65)and poor prognosis group(n=25).Their age,sex and other general data were collected,and the related risk factors affecting their prognosis were analyzed with logistic regression analysis model.Results:A total of 54 strains of pathogenic bacteria were cultured from 90 patients,including 42 strains of Gram negative bacteria,accounting for 77.78%.There were 5 strains of Gram positive bacteria,accounting for 9.26%.6 fungi accounts for 11.11%.1acid fast bacillus accounts for 1.85%.Logistic regression analysis showed that age≥60 years old,smoking history,diabetes,stroke,heart failure or myocardial infarction,mechanical ventilation,APAECHE-Ⅱ score≥24 were independent risk factors for the prognosis of patients with severe pneumonia(OR=58.286,12.250,21.000,10.444,7.125,8.381,8.941).Conclusion:Gram negative bacteria are the main pathogens in patients with severe pneumonia,and there are many factors affecting the prognosis,such as≥60 years old,smoking history,diabetes,stroke,heart failure or myocardial infarction,mechanical ventilation,etc.Therefore,it is suggested to attach great importance to it and take corresponding measures to control infection and improve the survival rate of patients.
