摘要
目的:本研究旨在观察Narcotrend监测下右美托咪定对丙泊酚抑制纤维结肠镜检查术内脏性疼痛半数有效浓度(EC50)的影响,为临床上个体化麻醉用药提供参考.方法:选取2022年1-12月华南理工大学附属第六医院收治的60例全麻下行纤维结肠镜检查术患者作为研究对象,采用随机数表法分为右美托咪定+丙泊酚试验组(D组)和生理盐水+丙泊酚对照组(C组),每组各30例.按Dixon序贯法采用Marsh药代动力学参数靶控输注丙泊酚.用概率单位法确定丙泊酚EC50、95％CI、麻醉并发症及苏醒时间.结果:D组患者丙泊酚EC50、95％CI低于C组,差异有统计学意义(P＜0.05);两组患者间麻醉并发症反应的发生率比较,差异无统计学意义(P＞0.05).结论:右美托咪定可明显降低丙泊酚抑制纤维结肠镜检查术内脏性疼痛的EC50.
Abstract
Objective:To observe the effect of dexmedetomidine on the inhibition of EC50 in patients with visceral pain by propofol under the monitoring of Narcotrend,so as to provide reference for individual anesthetic medication.Methods:60 patients underwent fibrocolonoscopy under general anesthesia in the hospital from January to December 2022 were selected as the research objects.They were divided into dexmedetomidine+propofol test group(group D)and saline+propofol control group(group C),with 30 patients in each group by randomized number method.Using Marsh pharmacokinetic parameters and target controlled infusion of propofol according to Dixon sequential method was performed.Propofol EC50,95％CI,complications of anesthesia and time of awakening were determined by probabilistic unit method.Results:EC50 and 95％CI of group D were lower than group C,with statistically significant difference(P＜0.05).There was no statistically significant difference in complications of anesthesia between the two groups(P＞0.05).Conclusion:Dexmedetomidine can significantly reduce propofol inhibition of EC50 for visceral pain during fibrocolonoscopy.
