Effect of Oral Helicobacter Pylori Infection on Inflammatory Factors in Gingival Crevicular Fluid and Minocycline Treatment in Patients with Chronic Periodontitis
Objective:To analyze the effect of oral helicobacter pylori infection(Hp)on inflammatory factors in gingival crevicular fluid(GCF)and minocycline treatment in patients with chronic periodontitis.Methods:A prospective cohort study was conducted to select 90 patients with chronic periodontitis who were admitted to Department of Stomatology of Xinyang People's Hospital from April 2019 to April 2022.All patients were tested for Hp before treatment,and were divided into infected group and uninfected group according to the infection condition,with 45 patients in each group.All patients received minocycline treatment for 4 weeks.The baseline data questionnaire was designed and the baseline data of patients were collected for comparison.The expressions of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-8(IL-8)in gingival creval fluid of the two groups were analyzed before treatment,and the clinical effects of the two groups were statistically analyzed after 4 weeks of treatment.Analytical logistic regression was used to analyze the effect of Hp infection on the treatment of minocycline in patients with chronic periodontitis.Results:The levels of TNF-α,IL-6 and IL-8 in gingival crevicular fluid in the infected group were significantly higher than those in the uninfected group before treatment,with statistically significant difference(t=32.221,9.742,5.533;P<0.05).After 30 days of minocycline treatment,the effective rate of the infected group was lower than that of the uninfected group,with statistically significant difference(x2=7.200,P<0.05).The levels of TNF-α,IL-6,IL-8 in gingival creval fluid and Hp infection in the ineffective treatment group were higher than those in the effective treatment group,and the differences were statistically significant(t=5.009,4.481,4.051;x2=7.200;P<0.05).Logistic regression analysis showed that high expression of TNF-α,IL-6,IL-8 in gingival crevicular fluid and Hp infection were risk factors for effective treatment of chronic periodontitis(OR=1.003,4.137,3.487,11.000;P<0.05).Conclusion:The levels of TNF-α,IL-6 and IL-8 in gingival crevicular fluid of patients with chronic periodontitis infected with Hp will increase significantly,and will reduce the therapeutic effect of minocycline.
