目的:探讨鳞状上皮细胞癌抗原(SCC)、细胞角蛋白19(CYFRA21-1)及血清应答因子(SRF)对食管癌(EC)淋巴结转移的诊断价值分析.方法:回顾性分析2019年2月—2021年1月洛阳市东方人民医院治疗的128例EC患者的临床资料,根据患者淋巴结转移情况将其分为淋巴结转移组和未发生淋巴结转移组,对比两组患者临床资料,分析EC患者SCC、CYFRA21-1、SRF与EC患者发生淋巴结转移的相关性;采用受试者工作特征(ROC)曲线下面积评价SCC、CYFRA21-1和SRF对食管癌淋巴结转移的诊断效能.结果:本研究纳入的128例EC患者中,有40例患者发生了淋巴结转移,淋巴结转移发生率为31.25%;与未发生淋巴结转移组相比,淋巴结转移组年龄、性别、体质量指数(BMI)、居住地、婚姻情况、教育程度、抽烟、饮酒、病变部位、肿瘤长径、高血压、糖尿病等临床资料对比,差异无统计学意义(t/x2=0.302、0.236、1.283、0.145、0.348、0.348、0.984、0.046、0.029、0.332、2.086、0.398、1.112,P>0.05),而 TNM 分期、肿瘤分化程度、SCC、CYFRA21-1、SRF等临床资料对比,差异有统计学意义(t/x2=6.188、5.593、7.435、7.160、8.503,P<0.05);经logistic回归分析显示,肿瘤分化程度、SCC、CYFRA21-1及SRF均是淋巴结转移的危险因素;Pearson相关性分析结果显示,SCC、CYFRA21-1、SRF表达水平与淋巴结转移的发生呈正相关(r=0.645、0.667、0.658,P<0.05);SCC、CYFRA21-1、SRF预测淋巴结转移的曲线下面积(AUC)分别为 0.666、0.860 及 0.883,而 SCC、CYFRA21-1、SRF三者联合检测预测淋巴结转移的AUC为0.952高于各指标单独检测,差异有统计学意义(P<0.05).结论:SCC、CYFRA21-1、SRF与淋巴结转移的发生具有较高的相关性,且SCC、CYFRA21-1、SRF三者联合检测淋巴结转移发生的效能优于各指标单独检测.
Diagnostic Value of Squamous Cell Carcinoma Antigen,Cytokeratin 19 and Serum Response Factor in Lymph Node Metastasis of Esophageal Cancer
Objective:To investigate the diagnostic value of squamous cell carcinoma antigen(SCC),cytokeratin 19(CYFRA21-1)and serum response factor(SRF)in lymph node metastasis of esophageal cancer(EC).Methods:The clinical data of 128 patients with esophageal cancer who were treated in Luoyang Oriental People's Hospital from February 2019 to January 2021 were retrospectively analyzed.According to the lymph node metastasis of the patients,they were divided into lymph node metastasis group and non lymph node metastasis group.Clinical data of the patients were compared.And the correlation between SCC,CYFRA21-1 and SRF in patients with esophageal cancer and lymph node metastasis in patients with esophageal cancer was analyzed.The area under receiver operating characteristic(ROC)curve was used to evaluate the diagnostic efficacy of SCC,CYFRA21-1 and SRF in lymph node metastasis of esophageal cancer.Results:Among the 128 patients with esophageal cancer included in this study,40 patients developed lymph node metastasis,and the incidence of lymph node metastasis was 31.25%.Compared with no lymph node metastasis group,there was no significant difference in age,sex,BMI,place of residence,marital status,education level,smoking,drinking,lesion site,tumor length,hypertension and diabetes in the group with lymph node metastasis(t/x2=0.302,0.236,1.283,0.145,0.348,0.348,0.984,0.046,0.029,0.332,2.086,0.398,1.112;P>0.05).There was statistically significant difference in TNM staging,tumor differentiation,SCC,CYFRA21-1 and SRF,etc(4/x2=6.188,5.593,7.435,7.160,8.503;P<0.05).Logistic regression analysis showed that the degree of tumor differentiation,SCC,CYFRA21-1 and SRF were all risk factors for lymph node metastasis(r=0.645,0.667,0.658;P<0.05).The area under the curve AUC of SCC,CYFRA21-1 and SRF for predicting lymph node metastasis was 0.666,0.860 and 0.883,respectively.The AUC of SCC,CYFRA21-1 and SRF for predicting lymph node metastasis was 0.952,which was higher than that of each index alone.The difference was statistically significant(P<0.05).Conclusion:SCC,CYFRA21-1 and SRF are highly correlated with the occurrence of lymph node metastasis.And the combined detection of SCC,CYFRA21-1 and SRF is better than the detection of each index alone.
