Diagnosis and Clinical Analysis of 12 Cases with Pulmonary Cryptococcosis
Objective:To investigate the pathological and clinical features of pulmonary cryptococcosis to improve the diagnosis and differential diagnosis.Methods:Clinical data,imaging material and pathologic changes of 12 patients with pulmonary cryptococcosis in the hospital from January,2020 to December,2021 were retrospectively analyzed.Results:Among 12 cases with pulmonary cryptococcosis,the disease was diagnosed by surgical operation in 3 cases,by percutaneous transthoracic lung biopsy in 8 cases,and by transbronchial biopsy in 1 case.Histopathological examination showed the presence of non-necrotizing granulomatous lesions,some round or circular-like bodies of varying sizes grayish red or transparent inside or outside histiocyte,and there was a thick refraction capsule,and the staining of slime carmine.Conclusion:The features in clinical and imaging findings of patients with pulmonary cryptococcosis are often confused with lung cancer.Lung biopsy not only can make differential diagnosis with tumor or non-tumor,but also can be used to find organisms directly.Percutaneous transthoracic lung biopsy is firstly recommended for diagnosis of pulmonary cryptococcosis.The differential diagnosis of pulmonary cryptococcosis with lung cancer needs to closely combinate with clinical manifestation.
