Effects of Two Surgical Methods on Postoperative Urodynamic Status and Erectile Function in Patients with Benign Prostatic Hyperplasia
Objective:To analyze and explore the effects of two surgical methods on postoperative urodynamic status and erectile function in patients with benign prostatic hyperplasia(BPH).Methods:80 patients with BPH admitted to the hospital from July 2021 to September 2022 were selected as observation subjects,and they were divided into enucleation group(n=39)and electric resection group(n=41)according to different surgical methods.The enucleation group underwent bipolar plasma enucleation,and the electric resection group was carried out bipolar plasma electric resection.Perioperative indicators,incidence rates of postoperative complications and urodynamic status,erectile function,symptom severity and quality of life score 3 months after surgery were compared between the two groups.Results:The surgical time,continuous bladder irrigation time,catheter indwelling time and hospital stay in enucleation group were significantly shorter than those in electric resection group,and the weight of excised glands was significantly heavier than that in electric resection group.The total incidence rate of postoperative complications in enucleation group was significantly lower than that in electric resection group(t=2.091,4.447,4.840,3.251,4.433;χ2=4.515;P<0.05).3 months after surgery,the bladder compliance and maximum urine flow rate were increased significantly,while the residual urine volume and maximum detrusor pressure were lower than those of electric resection group(P<0.05).6 months after surgery,the total incidence rate of erectile dysfunction in enucleation group(5.13%)was significantly lower than that in electric resection group(21.95%),with statistically significant difference(χ2=4.769,P<0.05).The symptom severity score and quality of life score 3 months after surgery were significantly decreased in both groups(P<0.05),and the scores were significantly lower in enucleation group than that in electric resection group(t=3.470,3.248;P<0.05).Conclusion:Compared with TUPKP,TUKEP had the advantages of minimally invasive property,rapid postoperative recovery and low incidence rates of complications in the treatment of BPH.TUKEP can effectively improve the clinical symptoms,urodynamic status and erectile dysfunction,and enhance the quality of life of patients.
