Application of Case Management-based Individualized Guidance in CRRT Fluid Management
Objective:To explore the application effects of case management-based individualized guidance in continuous renal replacement therapy(CRRT)fluid management.Methods:A total of 100 patients who underwent CRRT treatment from January 2020 to December 2021 were included in the study,and they were divided into observation group(n=44)and control group(n=56)according to the treatment time.Control group received routine CRRT nursing,and observation group added case management-based individualized guidance on this basis.The machine alarms,incidence rates of nursing risk events and nosocomial infections were compared between the two groups.The ventilation beginning time and CRRT treatment time were compared between the two groups,and the nursing satisfaction(patients,specialist nurses and assisting nurses)was evaluated.Results:The total frequency of alarms and frequency of pressure alarms in observation group were lower than those in control group,with statistically significant difference(x2=8.553,4.090;P<0.05).The incidence of failure to timely deal with alarm,failure to timely observe pressure index,failure to timely detect vital signs,CRRT observation form record defects and total nursing risk events in the observation group were lower than those in the control group,and the difference was statistically significant(x2=4.090,4.547,4.172,4.030,23.027;P<0.05).There was no significant difference in the incidence of nosocomial infection(x2=2.250,P=0.134).The time spent on the computer in the observation group was less than that in the control group,and the time spent on CRRT was more than that in the control group,with statistically significant difference(t=5.366,2.401;P<0.05).The total satisfaction rate of patients in the observation group was higher than that in the control group,the difference was statistically significant(x2=4.625,P<0.05),and the total satisfaction rate of specialist nurses and assistant nurses was not statistically significant between the two groups(x2=1.989,1.678;P>0.05).Conclusion:Case management-based individualized guidance has certain clinical application value in CRRT fluid management,and it is beneficial to the smooth progress of CRRT.
