Clinical Observation of Central Venous Catheter Thoracic Closed Drainage Combined with Urokinase in the Treatment of Separated Tuberculous Pleurisy
Objective:To investigate the therapeutic effects of administering Central Venous Catheter(CVC)combined with urokinase during the treatment of Tuberculous Pleurisy(TP)patients.Methods:54 patients who received separated TP treatment in hospital from September 2019 to September 2022 were randomly selected for study.The patients were divided into observation group and control group with 27 patients in each group.The treatment methods used in the control group were conventional anti-tuberculosis regimen and closed thoracic drainage with CVC,while the treatment methods used in the observation group were conventional anti-tuberculosis regimen and closed thoracic drainage with CVC combined with urokinase.Results:The treatment effect of observation group was significantly better than that of control group(x2=14.072,P<0.05).Compared with the control group,the disappearance of fluid,fever and hospitalization time in the observation group were relatively short,and the total amount of fluid extracted was relatively larger(t=4.303,2.447,3.182,2.306;P<0.05).The incidence of wrapped effusion,pleural reaction,pneumothorax and subcutaneous hematoma in observation group was lower than that in control group,with statistical significance(x2=18.316,16.923,12.694,11.075;P<0.05).Conclusion:The application of CVC thoracic closed drainage combined with urokinase in patients with separated TP can shorten the time of fluid disappearance,fever reduction and hospital stay;increase the total amount of pleural effusion extraction;and reduce the incidence of adverse reactions,with high safety and ideal therapeutic effect.
