Clinical Effect of Platelet-rich Plasma Combined with Screw Internal Fixation of Injured Vertebrae Through Wiltse Intermuscular Approach in the Treatment of Thoracolumbar Fracture
Objective:To observe the clinical effect of platelet-rich plasma(PRP)combined with screw internal fixation of injured vertebrae through wiltse intermuscular approach in the treatment of thoracolumbar fracture(TLF).Methods:A total of 84 patients with TLF admitted to the hospital from February 2022 to February 2023 were divided into control group(n=42)and observation group(n=42)with odd-even grouping method.The two groups were treated with screw internal fixation of injured vertebrae through Wiltse intermuscular approach.The observation group was treated with PRP injection on the basis of surgical treatment.The indexes related to operation,postoperative rehabilitation,inflammatory response and peroxidatic reaction were compared between the two groups.Results:The time of getting out of bed and the time of fracture healing were shorter in the observation group,with statistically significant difference(t=6.895,7.896;P<0.05).1 month,3 months and 6 months after surgery,the visual analogue scale(VAS)scores and Oswestry Disability Index(ODI)scores of the observation group were lower than the control group,with statistically significant difference(t=11.008,4.717,4.348,6.008,8.347,6.332;P<0.05).1 month after operation,the levels of interleukin-7 receptor(IL-7R),interleukin-6(IL-6)and malondialdehyde(MDA)in the two groups decreased,while the level of superoxide dismutase(SOD)increased,and the changes in the observation group were greater,with statistically significant difference(t=14.962,12.736,8.243,7.455;P<0.05).Conclusion:PRP combined with screw internal fixation of injured vertebrae through wiltse intermuscular approach for the treatment of TLF has good clinical effect,which can further improve the postoperative inflammatory response and peroxidatic reaction of patients,accelerate the postoperative recovery of patients,reduce postoperative pain,and promote the recovery of lumbar function.
