Clinical Efficacy and Impact on Neck Function of Non-Inflatable Axillary Approach Endoscopic Thyroid Surgery for Thyroid Cancer
Objective:To explore the clinical effect of endoscopic thyroid surgery without inflatable axillary approach in the treatment of thyroid cancer and its influence on neck function.Methods:A total of 160 patients with thyroid cancer admitted to The First Affiliated Hospital of Xiamen University from January 2023 to January 2024 were selected as the study objects.They were divided into control group(n=80,traditional open surgery)and observation group(n=80,non-inflatable axillary approach endoscopic thyroid surgery)according to surgical treatment methods.Clinical operation related indexes,neck function,parathyroid function and postoperative complications were compared between the two groups.Results:The total amount of drainage fluid,operation time and hospital stay in the observation group were significantly higher than those in the control group,and the number of lymph nodes dissection and intraoperative blood loss in the central region were significantly lower than those in the control group,with statistically significant difference(t=24.828,7.067,5.091,2.028,9.274;P<0.05).3 days and 6 months after operation,the dysphagia index scores of the observation group were significantly lower than those of the control group,and the difference was statistically significant(t=6.098,P<0.05).3 days after operation,serum parathyroid hormone(PTH)and blood calcium levels in 2 groups were decreased,and the observation group was significantly higher than the control group,with statistically significant difference(t=8.972,2.910;P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(x2=2.927,P>0.05).Conclusion:In patients with thyroid cancer,endoscopic thyroid surgery with non-inflatable axillary approach is comparable to open surgery in efficacy and safety.It has no significant difference from traditional open surgery in neck pain and neck injury,but has little impact on swallowing function.
