Correlation analysis and clinical significance of carotid ultrasound parameters and serum Annexin A2 level with the degree of coronary artery stenosis in patients with coronary heart disease
Objective:To investigate the correlation between serum Annexin A2(ANXA2)levels and carotid artery ultrasound parameters with the degree of coronary artery stenosis in patients with coronary heart disease,and to analyze its clinical significance.Methods:A retrospective study was conducted on 104 coronary heart disease patients from July 2021 to February 2024 at Xinzheng Huaxin Minsheng Hospital.The patients were divided into mild stenosis group(n=39)and moderate-to-severe stenosis group(n=65)based on the degree of coronary artery stenosis.Clinical general information,carotid artery ultrasound parameters[carotid artery stiffness(β),ca-rotid artery compliance(AC),elastic modulus(Ep),carotid intima-media thickness(IMT)]and serum ANXA2 levels were compared between the two groups,and their correlations were analyzed.The diagnostic efficacy and risk assessment of these parameters in severe coronary artery stenosis were also evaluated.Results:The left ventricular end-diastolic diameter(LVEDD)and left ventricular end-systolic diameter(LVESD)were higher,while the left ventricular ejection fraction(LVEF)was lower in the moderate-to-severe stenosis group compared to the mild stenosis group(P<0.05).The carotid artery ultrasound parameters of β,Ep,AC,IMT and serum ANXA2 levels were higher in the mild stenosis group than in the moderate-to-severe stenosis group(P<0.05).The pa-rameters of β,Ep,AC,IMT,ANXA2 were positively correlated with echocardiographic parameters LVEDD and LVESD,and negatively correlated with LVEF(P<0.05).The area under the curve(AUC)for the combined di-agnosis of moderate to severe stenosis using carotid ultrasound parameters and serum ANXA2 levels at admission was 0.784,with optimal sensitivity and specificity of 76.92%and 78.46%,respectively.The risk of adverse prognosis was 1.728,1.741,1.898,1.929 and 1.955 times higher in patients with high levels of carotid artery ultrasound parameters and serum ANXA2 at admission compared to those with low levels(P<0.05).Conclu-sion:Serum ANXA2 levels and carotid artery ultrasound parameters are correlated with the severity of coronary heart disease and can serve as effective tools for early assessment of disease severity.
