Therapeutic effect of HF-rTMS combined with memantine hydrochloride and nimodipine in the treatment of vascular dementia and its impact on endothelial cell function
Objective To investigate the efficacy of high-frequency repetitive transcranial magnetic stimulation(HF-rTMS)combined with memantine hydrochloride and nimodipine in the treatment of vascular dementia and the effect on endothelial cell function.Methods Sixty-eight patients with vascular dementia admitted to the Department of Neurology,Affiliated People's Hospital of Shandong First Medical University from February 2022 to February 2023 were included in this study and were randomly divided into control group and study group.The control group was treated with memantine hydrochloride and nimodipine.The study group added HF-rTMS treatment on the basis of the control group,and the clinical efficacy(including cognitive function and daily living ability before and after treatment)and the influence on endothelial cell function(endothelin(ET1),soluble intercellular adhesion molecule-1(sICAM-1))of the two groups were compared.Results Before treatment,there was no significant difference on MMSE score and ADL score between the two groups(t=0.646,0.080;P=0.521,0.936).After treatment,the scores of the two groups were improved,MMSE and ADL scores in the study group were higher than those in the control group(t=4.755,P<0.001;t=6.224,P<0.001).Before treatment,there was no statistical significance in the levels of ET1 and sICAM-1 between the two groups(t=0.233,0.344;P=0.817,0.732),but after treatment,the levels of ET1 and sICAM-1 in the study group were lower than those in the control group(t=4.781,P<0.001;t=2.400,P=0.0 19).In the study group,2 patients(5.89%)experienced dizziness after HF-rTMS treatment,and the symptoms disappeared after adjusting the stimulation intensity.Conclusion HF-rTMS combined with memantine hydrochloride and nimodipine can improve cognitive function,daily living ability and endothelial cell function in patients with vascular dementia,and the combination therapy is safe.
