Objective To investigate the application value of thrombelastogram(TEG)in the antiplatelet therapy after intracranial aneurysm(IA)embolization.Methods Ninety-eight patients who received 1A embolization in the Nanchong Central Hospital,North Sichuan Medical College from January 2021 to May 2023 were collected and divided into control group(n=32,standard dual antiplatelet therapy)and observation group(n=66,TEG-guided antiplatelet therapy)according to whether they received TEG guidance.Complications,platelet aggregation inhibition rate,coagulation function,neurological function and prognosis were compared between the two groups.Results The postoperative bleeding rate in observation group was 1.52%,which was significantly lower than 15.63%in control group(P<0.05).The postoperative ischemia rate with 22.73%in observation group was slightly higher than 21.88%in control group,but there was no significant difference(P>0.05).The inhibition rates of arachidonic acid pathway and adenosine diphosphate pathway in observation group were significantly higher compared to control group(P<0.05).The platelet level and NIHSS score in observation group were significantly lower than those in control group(P<0.05).There was no statistical difference in Glasgow outcome scale score between the two groups(P>0.05).Conclusion The application of antiplatelet therapy under TEG guidance in patients after IA embolization can provide personalized guidance for dose adjustment of antiplatelet therapy,reduce the occurrence of bleeding events and improve the neurological function.
