摘要
目的 探讨血管生成素样蛋白4(ANGPTL4)、凝血酶敏感蛋白-1(TSP-1)、亲环素A(CyPA)与脑卒中后癫痫患者认知功能的关系.方法 选取2021-01-2022-12河南大学第一附属医院神经内科收治的100例脑卒中后癫痫病例进行观察,按简易精神状态量表(MMSE)划分认知障碍标准将患者分为认知障碍组(50例)和认知正常组(50例),应用酶联免疫吸附试验(ELISA)检测2组患者的血清ANGPTL4、TSP-1、CyPA水平,MMSE量表测评2组患者的认知功能.结果 与认知正常组比较,认知障碍组患者MMSE评分降低,ANGPTL4、TSP-1、CyPA水平升高(P＜0.05);与轻度认知障碍患者比较,中度认知障碍患者血清ANGPTL4、TSP-1、CyPA水平升高(P＜0.05);与中度认知障碍患者比较,重度认知障碍患者血清ANGPTL4、TSP-1、CyPA水平升高(P＜0.05).在脑卒中后癫痫患者中,血清ANGPTL4、TSP-1、CyPA与MMSE评分各维度均呈负相关(P＜0.05).结论 脑卒中后癫痫会降低MMSE评分,提高患者血清ANGPTL4、TSP-1、CyPA水平.ANGPTL4、TSP-1、CyPA水平越高,患者认知功能障碍越严重.
Abstract
Objective To investigate the relationship between angiopoietin like protein 4(ANGPTL4),thrombospondin-1(TSP-1),cyclophilin A(CyPA)and cognitive function of epilepsy patients after stroke.Methods One hundred post-stroke epilepsy patients admitted to the Neurology Department of the First Affiliated Hospital of Henan University from January 2021 to December 2022 were divided into cognitive impairment group(50 cases)and cognitive normal group(50 cases)based on the results of the mini-mental state scale(MMSE).The serum levels of ANGPTL4,TSP-1,and CyPA were measured by enzyme-linked immunosorbent assay(ELISA)in both groups.The MMSE scale was used to assess cognitive function.Results Compared with the cognitive normal group,the MMSE score of patients with cognitive impairment decreased,while the levels of ANGPTL4,TSP-1,and CyPA increased(P＜0.05).Compared with mild cognitive impairment patients,moderate cognitive impairment patients had elevated serum levels of ANGPTL4,TSP-1,and CyPA(P＜0.05),while severe cognitive impairment patients had elevated serum levels of ANGPTL4,TSP-1,and CyPA(P＜0.05).In post-stroke epilepsy,serum ANGPTL4,TSP-1,CyPA,and MMSE scores were negatively correlated in all dimensions(P＜0.05).Conclusion Epilepsy after stroke can reduce MMSE scores and increase serum levels of ANGPTL4,TSP-1,and CyPA in patients.The higher of the levels of ANGPTL4,TSP-1,and CyPA,the more severe of the cognitive impairment.
