摘要

目的 探讨光谱CT不同单能量图像在头部CT检查患者的应用并进行效果对比.方法 选取2023-06-2024-01因头晕、晕厥、头痛等临床症状就诊的98例患者,所有患者均接受光谱CT头部检查.根据CT检查方法分为单能量组(n=51)和混合能量组(n=47).单能量组采用单能量图像(40~120 keV),根据编入不同的管电压情况,以5 keV为间隔增量,重建为17组不同单能量图像,混合能量组采用常规混合能量图像.比较单能量组和混合能量组脑血管CT值(Willis环、大脑前动脉、大脑中动脉、大脑后动脉)、不同解剖平面下的噪声(胼胝体干部、四叠体池层面、鞍上池层面、垂体层面)、图像硬化伪影区域[CT值、标准差(SD)、信噪比(SNR)、噪声比(CNR)],以及不同单能量图像的图像硬化伪影区域参数.结果 单能量组关于Willis环、大脑前动脉、大脑中动脉、大脑后动脉参数值均高于混合能量组(P<0.05).单能量组关于胼胝体干部、四叠体池层面、鞍上池层面、垂体层面解剖平面下的噪声参数值均低于混合能量组(P<0.05).单能量组关于硬化伪影区域CT值高于混合能量组,SD、SNR和CNR值低于混合能量组(P<0.05).在40~120 keV单能量图像中,120 keV图像硬化伪影区域的CT值高于40 keV,SD、SNR、CNR参数值低于40 keV.结论 光谱CT可提高头部CT检查患者图像脑血管CT值,降低图像噪声,尤以120 keV图像质量最佳.

Abstract

Objective To investigate the application and effect comparison of different single energy images of spectral CT in patients undergoing head CT examination.Methods A total of 98 patients due to clinical symptoms(dizziness,syncope,headache)between June 2023 and January 2024 were enrolled.All patients underwent spectral CT head examination.According to different CT examination methods,they were divided into single energy group(n=51)and mixed energy group(n=47).In single energy group,single energy images(40-120 keV)were reconstructed into 17 groups of different single energy images in increments of 5 keV according to different tube voltages,while routine mixed energy images were applied in mixed energy group.CT values of cerebral vessels(Willis loop,anterior cerebral artery,middle cerebral artery,posterior cerebral artery),noises in different anatomical planes(truncus corporis callosi,quadrigeminal cisterna layer,suprasellar cisterna layer,pituitary layer)and image hardening artifact areas(CT value,standard deviation(SD),signal-to-noise ratio(SNR),noise ratio(CNR))were compared between the two groups.The parameters of image hardening artifact areas in different single energy images were compared.Results The parameters of Willis loop,anterior cerebral artery,middle cerebral artery and posterior cerebral artery in single energy group were higher than those in mixed energy group(P<0.05),noise values of truncus corporis callosi,quadrigeminal cisterna layer,suprasellar cisterna layer and pituitary layer were lower than those in mixed energy group(P<0.05).CT value of hardening artifact area in single energy group was greater than that in mixed energy group,while SD,SNR and CNR were lower than those in mixed energy group(P<0.05).In 40-120 keV single energy images,CT value of hardening artifact area in 120 keV images was greater than that in 40 keV images,while SD,SNR and CNR were lower than those in 40 keV images.Conclusion Spectral CT can increase CT values of cerebral vessels and reduce image noise in patients undergoing head CT examination,especially in 120 keV images.
