2型糖尿病合并急性脑梗死患者甲状腺激素水平与病情严重程度的相关性

[目的]探讨 2 型糖尿病(T2DM)合并急性脑梗死(ACI)患者甲状腺激素水平与病情严重程度相关性.[方法]选取 2019 年 1 月至 2022 年 1 月本院收治的 80 例 T2DM合并 ACI患者(A组),另选取本院同期收治 80例单纯T2DM患者为B组.两组均在入院后检测血糖指标[空腹血糖(FPG)、日内平均血糖波动幅度(MAGE)、血糖水平标准差(SDBG)]、甲状腺激素[三碘甲腺原氨酸(T3)、甲状腺素(T4)、促甲状腺激素(TSH)]水平并比较.采用美国国立卫生研究院卒中量表(NIHSS)评估 A组患者神经缺损程度,分为轻度组(n =27,NIHSS评分≤8 分)、中度组(n =32,NIHSS评分 9~14 分)、重度组(n =21,NIHSS评分≥15 分),比较不同病情严重程度T2DM合并ACI患者血糖及甲状腺激素差异,分析血糖、甲状腺激素与T2DM合并ACI病情严重程度的相关性.治疗后 3 个月,采用改良 Rankin评分量表评估 A组患者神经功能恢复情况,分为预后良好组(n =50,0~1 级为预后良好)及预后不良组(n =30,2~6 级为预后不良),比较 A 组不同预后患者血糖及甲状腺激素相关指标差异,分析上述指标对 T2DM合并 ACI 患者预后的影响.[结果]A 组 FPG、MAGE、SDBG、TSH 水平显著高于 B组(P<0.05),A组T3 水平显著低于B组(P<0.05);两组T4 水平比较,差异无统计学意义(P>0.05).轻度组、中度组、重度组 FPG、MAGE、SDBG、TSH 水平依次升高(P<0.05),T3 水平依次降低(P<0.05);三组 T4水平组间比较,差异无统计学意义(P>0.05).FPG、MAGE、SDBG、TSH 与病情严重程度呈正相关(P<0.05),T3 与病情严重程度呈负相关(P<0.05).预后良好组 FPG、MAGE、SDBG、TSH 水平显著低于预后不良组,T3 水平显著高于预后不良组(P<0.05);两组 T4 水平比较,差异无统计学意义(P>0.05).[结论]T2DM合并 ACI患者病情越严重,体内 FPG、MAGE、SDBG、TSH 水平越高,T3 水平越低.
Correlation between Thyroid Hormone Level and Severity of Disease in Patients with Type 2 Diabe-tes Mellitus Complicated with Acute Cerebral Infarction
[Objective]To explore the correlation between thyroid hormone level and severity of type 2 diabe-tes mellitus(T2DM)complicated with acute cerebral infarction(ACI).[Methods]A total of 80 patients with T2DM complicated with ACI admitted to our hospital from January 2019 to January 2022 were selected as group A,and 80 patients with simple T2DM admitted to our hospital during the same period were selected as group B.After admission,blood glucose indexes[fasting blood glucose(FPG),mean daily blood glucose fluctuation(MAGE),standard deviation of blood glucose level(SDBG)]and thyroid hormone[triiodothyronine(T3),thyrox-ine(T4),thyroid stimulating hormone(TSH)]levels were detected and compared between the two groups.The National Institutes of Health Stroke Scale(NIHSS)was used to evaluate the degree of neurological deficit in group A patients.The patients were divided into mild group(n =27,NIHSS score≤8),moderate group(n =32,NIHSS score 9-14),and severe group(n =21,NIHSS score≥15).The differences of blood glucose and thyroid hormone in T2DM patients with ACI were compared,and the correlation between blood glucose,thyroid hormone and the severity of T2DM patients with ACI was analyzed.Three months after treatment,the improved Rankin scale was used to evaluate the neurological recovery of patients in group A,and they were divided into good prog-nosis group(n =50,grade 0-1 for good prognosis)and poor prognosis group(n =30,grade 2-6 for poor prog-nosis).The differences of blood glucose and thyroid stimulating hormone related indicators in patients with differ-ent prognosis in group A were compared,and the influence of the above indicators on the prognosis of T2DM pa-tients with ACI was analyzed.[Results]The levels of FPG,MAGE,SDBG and TSH in group A were significantly higher than those in group B(P<0.05),and the level of T3 in group A was significantly lower than that in group B(P<0.05).There was no significant difference in the level of T4 between the two groups(P>0.05).The levels of FPG,MAGE,SDBG and TSH in the mild group,the moderate group and the severe group increased in turn(P<0.05),and the level of T3 decreased in turn(P<0.05).There was no significant difference in T4 levels among the three groups(P>0.05).FPG,MAGE,SDBG,TSH were positively correlated with the severity of the disease(P<0.05),T3 was negatively correlated with the severity of the disease(P<0.05).The levels of FPG,MAGE,SDBG and TSH in the good prognosis group were significantly lower than those in the poor prognosis group,and the level of T3 in the good prognosis group was significantly higher than that in the poor prognosis group(P<0.05).There was no significant difference in the level of T4 between the two groups(P>0.05).[Con-clusion]The more severe the condition of T2DM patients with ACI,the higher the levels of FPG,MAGE,SDBG,TSH,and the lower the level of T3.
