HR-HPV DNA、LPT检测对宫颈鳞状细胞癌患者的诊断价值

[目的]探讨高危人乳头瘤病毒(HR-HPV)脱氧核糖核酸(DNA)联合液基薄层细胞学(LPT)检测对宫颈鳞状细胞癌的诊断价值.[方法]选取 2019 年 5 月至 2021 年 4 月在西安高新医院诊治的 84 例宫颈鳞状细胞癌患者(宫颈鳞状细胞癌组)和 60 例慢性宫颈炎患者(慢性宫颈炎组),另外选择同期在西安高新医院进行体检的50 例健康女性为对照组.比较三组 HR-HPV DNA、LPT阳性率;比较不同分期、不同分化程度宫颈鳞状细胞癌患者 HR-HPV DNA、LPT阳性率.以病理检查结果为金标准,分析 HR-HPV DNA 联合 LPT 检测对宫颈鳞状细胞癌的诊断效能.[结果]宫颈鳞状细胞癌组和慢性宫颈炎组HR-HPV DNA、LPT阳性率均高于对照组(P<0.05);宫颈鳞状细胞癌组 HR-HPV DNA、LPT阳性率高于慢性宫颈炎组(P<0.05).不同分期宫颈鳞状细胞癌患者 HR-HPV DNA、LPT阳性率比较,差异均无统计学意义(P>0.05);不同分化程度宫颈鳞状细胞癌患者HR-HPV DNA、LPT阳性率比较,差异均无统计学意义(P>0.05).受试者工作特征(ROC)曲线分析结果显示:HR-HPV DNA联合LPT检测诊断宫颈鳞状细胞癌的灵敏度显著高于 HR-HPV DNA、LPT单独检测的灵敏度;HR-HPV DNA联合 LPT检测诊断宫颈鳞状细胞癌的 AUC 高于 HR-HPV DNA、LPT 单独检测的 AUC(Z = 3.940,P<0.001;Z =3.968,P<0.001).[结论]HR-HPV DNA联合 LPT检测诊断宫颈鳞状细胞癌的效能较高,可作为临床诊断宫颈鳞状细胞癌的重要参考指标.
Diagnostic Value of HR-HPV DNA and LPT Detection in Patients with Cervical Squamous Cell Car-cinoma
[Objective]To investigate the diagnostic value of high-risk human papillomavirus(HR-HPV)de-oxyribonucleic acid(DNA)combined with liquid based thin-prep cytology(LPT)in cervical squamous cell carcino-ma.[Methods]A total of 84 patients with cervical squamous cell carcinoma(cervical squamous cell carcinoma group)and 60 patients with chronic cervicitis(chronic cervicitis group)who were diagnosed and treated in Xi'an High-tech Hospital from May 2019 to April 2021 were selected,and 50 healthy women who underwent physical examination in the Xi'an High-tech Hospital during the same period were selected as the control group.The posi-tive rates of HR-HPV DNA and LPT were compared among the three groups;The positive rates of HR-HPV DNA and LPT in patients with cervical squamous cell carcinoma at different stages and different degrees of differ-entiation were compared.Taking the pathological examination results as the gold standard,the diagnostic efficacy of HR-HPV DNA combined with LPT detection for cervical squamous cell carcinoma was analyzed.[Results]The positive rates of HR-HPV DNA and LPT in the cervical squamous cell carcinoma group and the chronic cervicitis group were higher than those in the control group(P<0.05);The positive rates of HR-HPV DNA and LPT in cervical squamous cell carcinoma group were higher than those in the chronic cervicitis group.There was no signifi-cant difference in the positive rates of HR-HPV DNA and LPT in patients with cervical squamous cell carcinoma at different stages(P>0.05);There was no significant difference in the positive rates of HR-HPV DNA and LPT in patients with cervical squamous cell carcinoma of different differentiation degrees(P>0.05).Receiver operating characteristic(ROC)curve analysis showed that the sensitivity of HR-HPV DNA combined with LPT in the diag-nosis of cervical squamous cell carcinoma was significantly higher than that of HR-HPV DNA and LPT alone;The AUC of HR-HPV DNA combined with LPT in the diagnosis of cervical squamous cell carcinoma was higher than that of HR-HPV DNA and LPT alone(Z =3.940,P<0.001;Z =3.968,P<0.001).[Conclusion]HR-HPV DNA combined with LPT is highly effective in the diagnosis of cervical squamous cell carcinoma,and can be used as an important reference index for clinical diagnosis of cervical squamous cell carcinoma.
