[目的]探讨多层螺旋CT(MSCT)灌注扫描联合血清微小RNA-200b-3p(miR-200b-3p)、微小RNA-181b-5p(miR-181b-5p)对前列腺增生(BPH)和前列腺癌的鉴别价值.[方法]选取2021年5月至2023年9月本院收治的249例前列腺病变患者,根据病理结果将其分为BPH组(n=144)和前列腺癌组(n=105).采用Lo-gistic 回归分析血清miR-200b-3p、miR-181b-5p及MSCT灌注参数与前列腺癌的关系,绘制受试者工作特征曲线分析MSCT灌注参数联合血清miR-200b-3p、miR-181b-5p对前列腺癌的诊断价值.[结果]前列腺癌组患者血清miR-200b-3p水平高于BPH组,血清miR-181b-5p水平低于BPH组(P<0.05).前列腺癌组患者相对血流量(rBF)、表面渗透性(PS)显著高于BPH组(P<0.05).rBF、PS、miR-200b-3p、miR-181b-5p单独及联合诊断前列腺癌的曲线下面积分别为 0.780、0.760、0.763、0.801、0.932.Logistic 回归分析结果显示:miR-200b-3p>1.23、rBF>50.54 mL/(min·100 g)、PS>28.82 mL/(min·100 g)是影响前列腺癌发生的独立危险因素(P<0.05);miR-181b-5p<0.90是影响前列腺癌发生的保护因素(P<0.05).[结论]MSCT灌注成像参数联合血清miR-200b-3p、miR-181b-5p检测可为鉴别诊断BPH和前列腺癌提供可靠依据.
Differential Diagnostic Value of Peripheral Blood miR-200b-3p and miR-181b-5p Combined with Multi-Slice Spiral CT Perfusion Scan for Benign Prostatic Hyperplasia and Prostate Cancer
[Objective]To explore the differential value of multi-slice spiral CT(MSCT)perfusion scan combined with serum microRNA-200b-3p(miR-200b-3p)and microRNA-181b-5p(miR-181b-5p)in benign prostatic hyperplasia(BPH)and prostate cancer.[Methods]A total of 249 patients with prostate lesions admitted to our hospital from May 2021 to September 2023 were selected and divided into BPH group(n=144)and prostate cancer group(n=105)based on pathological results.Logistic regression analysis was used to investigate the relationship between serum miR-200b-3p,miR-181b-5p,and MSCT perfusion parameters with prostate cancer.A receiver operating characteristic curve was plotted to analyze the diagnostic value of MSCT perfusion parameters combined with serum miR-200b-3p and miR-181b-5p for prostate cancer.[Results]The serum miR-200b-3p levels in the prostate cancer group were higher than those in the BPH group,while the serum miR-181b-5p levels were lower than those in the BPH group(P<0.05).The rBF and PS of pa-tients in the prostate cancer group were significantly higher than those in the BPH group(P<0.05).The area under the curve of rBF,PS,miR-200b-3p,and miR-181b-5p for single and combined diagnosis of prostate cancer were 0.780,0.760,0.763,0.801,and 0.932,respectively.The results of logistic regression analysis showed that rBF>50.54 mL/(min·100 g),PS>28.82 mL/(min·100 g),miR-200b-3p>1.23 were independent risk factors affecting the occurrence of prostate cancer(P<0.05);MiR-181b-5p<0.90 is a protective factor affecting the occurrence of prostate cancer(P<0.05).[Conclusion]The combination of MSCT perfusion imaging parameters and serum miR-200b-3p and miR-181b-5p detection can provide reliable basis for the differential diagnosis of BPH and prostate cancer.
