Risk Factors of Deep Vein Thrombosis and Construction of the Prediction Model in Patients with Acute Ischemic Stroke
Objective To explore the risk factors of deep vein thrombosis(DVT)in patients with acute ischemic stroke(AIS),and to construct risk prediction model.Methods A retrospective analysis was performed on the relevant data of 415 patients with AIS admitted to Zhumadian Central Hospital between September 2020 and August 2023.According to presence or absence of DVT,they were divided into DVT group(81 cases)and non-DVT group(334 cases).The data in the two groups were compared.The risk factors of DVT were analyzed,and the risk prediction model was constructed.Results Univariate analysis and multivariate logistic analysis showed that age,usage of dehydrating drugs,bed rest and D-dimer were risk factors of DVT(P<0.05),but history of thrombosis,stroke severity and disturbance of consciousness were not(P>0.05).The risk prediction model was as follow:P=1/[1+exp(10.561-0.1 08 X1-0.920X2+1.038X3-0.009X4)],in which X1 was age,X2 was the application of dehydrating drugs,X3 was bed rest,X4 was D-dimer.The model was of statistical significance(x2=109.085),with high goodness of fit(Hosmer-Lemeshow test:P=0.368).The area under the curve was 0.839(95%CI:0.793-0.885).When there was the maximal Youden index,the sensitivity and specificity were 84.0%and 68.9%,respectively.And the risk probability value was 0.847.Conclusion Age,usage of dehydrating drugs,bed rest and D-dimer level are risk factors of DVT in AIS patients.The risk prediction model based on these factors has excellent predictive efficiency,which is beneficial to identify high-risk groups with DVT after AIS,and can provide reference for the prevention and treatment of DVT.
