Occurrence and Influencing Factors of Postoperative Chylothorax in Patients with Esophageal Cancer
Objective To explore the possible influencing factors of postoperative chylothorax in patients with esophageal cancer,in order to reduce the risk of postoperative chylothorax in patients with clinical esophageal cancer.Methods A total of 830 patients with esophageal cancer admitted to the hospital from January 2019 to December 2021 were enrolled.The patients were divided into groups according to whether or not they developed chylothorax after surgery,and the baseline data of the occurrence group and the non-occurrence group were compared to explore the factors affecting the postoperative occurrence of chylothorax in patients with esophageal cancer.Results There were 30 cases of chylothorax in 830 patients with esophageal cancer after operation,accounting for 3.61%(30/830),which were included in the occurrence group.There were 800 cases without chylothorax after operation,accounting for 96.39%(800/830),and they were included in the non-occurrence group.The proportion of tumor diameter(≥5 cm),tumor location(middle and upper segment),mediastinal lymph node enlargement(yes)and tumor stage(≥ Ⅲ)in the occurrence group was higher than that in the non-occurrence group(P<0.05).Multiple logistic regression analysis results showed that tumor diameter,tumor lesion location,mediastinal lymph node enlargement and tumor stage were all risk factors for chylothorax after esophageal cancer(OR>1,P<0.05).Conclusion Tumor diameter ≥5 cm,tumor lesion location in the upper middle segment,mediastinal lymph node enlargement,and tumor stage≥Ⅲ are all factors that affect the occurrence of postoperative chylothorax in esophageal cancer patients.Targeted intervention measures can be developed in clinical practice to reduce the risk of postoperative chylothorax in esophageal cancer patients with the combination of these risk factors.
