Evaluation Effects of Transcranial Doppler Ultrasound Foaming Test Combined with Acoustic Contrast in Interventional Closure of Patients with Patent Foramen Ovale
Objective To study the evaluation value of transcranial Doppler ultrasound(TCD)foaming test combined with acoustic contrast in the interventional closure of patients with patent foramen ovale.Methods Retrospective analysis was conducted on data of 124 patients diagnosed with highly suspected patent foramen ovale at the Department of Ultrasound of Zhengzhou Central Hospital Affiliated to Zhengzhou University from March 2020 to December 2021.All patients completed transesophageal echocardiography(TEE),TCD foaming test and acoustic contrast examination.As there was no clear gold standard for diagnosing patent foramen ovale currently,according to core literature and previous studies,TEE was used as a feasible control for diagnosing patent foramen ovale.Interventional closure was performed on patients who were diagnosed with patent foramen ovale and met the criteria of interventional closure.The efficacy of interventional closure was evaluated by TCD foaming test combined with acoustic contrast examination,and the postoperative recurrence of patients was recorded.Results Among 124 patients,115 were diagnosed with patent foramen ovale by TEE,with a detection rate of 92.74%.Compared with TEE,the sensitivity of TCD foaming test combined with acoustic contrast in diagnosing patent foramen ovale was 98.26%(113/115),the specificity was 77.78%(7/9),and the accuracy was 96.77%(120/124).The consistency K value between the joint examination diagnosis and TEE was 0.760,indicating good consistency(P<0.05).Among the 115 patients,113 met the indications for interventional closure surgery and successfully completed the treatment.After joint examination and evaluation,109 patients had effective surgery,with an effective rate of 96.46%.All 113 patients completed postoperative follow-up,and there were no deaths during the follow-up period.Two patients experienced recurrence,with a recurrence rate of 1.77%.Conclusion The TCD foaming experiment combined with acoustic contrast examination has high accuracy in diagnosing patent foramen ovale,and by accurately evaluating the surgical effect of interventional closure,it reduces postoperative recurrence caused by misjudgment of surgical effect,ensuring the effectiveness and accuracy of interventional closure treatment.
