目的 探讨高分辨率CT联合血清红细胞沉降率(ESR)、超敏C反应蛋白(hs-CRP)水平检测对尘肺合并肺结核的诊断效能.方法 选取医院2021年4月至2023年4月接收的已确诊为尘肺的112例患者作为研究对象,均接受高分辨率CT检查,以痰结核菌培养以及结核菌素(PPD)试验结果作为金标准,分析高分辨率CT对于尘肺合并肺结核的诊断标准,对比单纯尘肺、尘肺合并肺结核患者血清ESR、hs-CRP水平,分析高分辨率CT联合血清ESR、hs-CRP水平对尘肺合并肺结核的诊断效能以及不同血清ESR、hs-CRP水平尘肺患者发生肺结核的危险度.结果 112例尘肺患者中肺结核发生率为40.18%(45/112);高分辨率CT诊断敏感性、特异性、准确性、阳性预测值与阴性预测值依次为91.11%、85.57%、88.39%、82.00%、91.11%;尘肺患者hs-CRP、ESR均低于尘肺合并肺结核患者(P<0.05);受试者工作特征(ROC)曲线显示,hs-CRP、ESR、高分辨率CT联合诊断尘肺合并肺结核的曲线下面积(AUC)为0.814(95%CI:0.764~0.867),最佳诊断敏感性、特异性依次为93.33%、89.55%;高水平ESR、hs-CRP尘肺患者发生肺结核的危险度为低水平的0.472倍、0.403倍(P<0.05).结论 高分辨率CT联合血清hs-CRP、ESR水平在诊断尘肺合并肺结核中具有较高应用价值,可为临床早期诊断与早期治疗提供依据.
Diagnostic Efficacy of High-resolution CT Combined with Serum ESR and hs-CRP Levels Detection in Pneumoconiosis Combined with Pulmonary Tuberculosis
Objective To investigate the diagnostic efficiency of high resolution CT combined with serum erythrocyte sedimentation rate(ESR)and hypersensitive C-reactive protein(hs-CRP)on pneumoconiosis complicated with pulmonary tuberculosis.Methods A total of 112 patients diagnosed with pneumoconiosis received in the hospital from April 2021 to April 2023 were selected as research objects.All of them underwent high-resolution CT examination.The results of sputum tuberculin culture and purified protein derivative of tuberculin(PPD)test were used as the gold standard to analyze the diagnostic criteria of high-resolution CT for pneumoconiosis complicated with tuberculosis.Serum ESR and hs-CRP levels in patients with pneumoconiosis and pneumoconiosis combined with pulmonary tuberculosis were compared,and the diagnostic efficiency of high-resolution CT combined with serum ESR and hs-CRP levels in detecting pneumoconiosis combined with pulmonary tuberculosis was analyzed,and the risk of pulmonary tuberculosis in pneumoconiosis patients with different serum ESR and hs-CRP levels were analyzed.Results The incidence of pulmonary tuberculosis in 112 pneumoconiosis patients was 40.18%(45/112).The diagnostic sensitivity,specificity,accuracy,positive predictive value and negative predictive value of high-resolution CT were 91.11%,85.57%,88.39%,82.00%,91.11%,respectively.hs-CRP and ESR in pneumoconiosis patients were lower than those in pneumoconiosis patients with pulmonary tuberculosis(P<0.05).Receiver operating characteristics(ROC)curve showed that the area under the curve(AUC)value of hs-CRP,ESR and high-resolution CT in the diagnosis of pneumoconiosis complicated with pulmonary tuberculosis was 0.814(95%CI:0.764-0.867),and the optimal diagnostic sensitivity and specificity were 93.33%and 89.55%,respectively.The risk of pulmonary tuberculosis in patients with high ESR and hs-CRP was 0.472 times and 0.403 times that of low ESR and HS-CRP(P<0.05).Conclusion High-resolution CT combined with serum hs-CRP and ESR levels has high application value in the diagnosis of pneumoconiosis complicated with pulmonary tuberculosis,which can provide scientific data support for early clinical diagnosis and treatment.
