Correlation of Serum DAO,VIP and PCT Levels with Acute Ileus and the Diagnostic Value of Combined Detection in Strangulated Ileus
Objective To investigate the correlation between the levels of serum diamine oxidase(DAO),vasoactive intestinal peptide(VIP)and procalcitonin(PCT)in patients with acute ileus,and to analyze their diagnostic value for stranded ileus.Methods A total of 102 patients with acute ileus admitted to the hospital from May 2019 to May 2023 were retrospectively selected as the study group,and 34 healthy volunteers who underwent physical examination in the hospital during the same period were selected as the control group.Serum levels of DAO,VIP and PCT were detected and compared between the two groups.The study group underwent intestinal resection and was divided into mild injury(60 cases)and severe injury(42 cases)according to the intraoperative intestinal tube defect.The serum levels of DAO,VIP and PCT were compared at admission,1 day and 3 days after surgery.According to postoperative diagnosis,the study group was divided into simple intestinal obstruction(53 cases)and strangulated intestinal obstruction(49 cases).The clinical data and serum DAO,VIP and PCT levels at admission were compared.The influencing factors of strangulation intestinal obstruction were analyzed by logistic regression.Receiver operating characteristic(ROC)curve and area under curve(AUC)were used to analyze the diagnostic value of serum DAO,VIP and PCT levels in strangulated ileus.Results The levels of DAO,VIP and PCT in the study group were higher than those in the control group(P<0.05).The levels of DAO,VIP and PCT in patients with severe intestinal duct injury were higher than those in patients with mild intestinal duct injury at admission,1 and 3 days after surgery,and the levels showed a decreasing trend with the extension of postoperative time(P<0.05).The levels of DAO,VIP and PCT in patients with strangulated ileus were higher than those in patients with simple ileus(P<0.05).The history of abdominal operation and the levels of DAO,VIP and PCT in serum at admission were independent risk factors for strangulation intestinal obstruction(P<0.05).The AUC of the combined detection of DAO,VIP and PCT was higher than that of the combined detection of any two indexes,and higher than that of the single index(P<0.05).Conclusion Serum levels of DAO,VIP and PCT are elevated in patients with acute ileus.The combined detection of DAO,VIP and PCT levels has certain diagnostic value for strangulated ileus,and may be used as an auxiliary index to evaluate the severity of the disease.
