经阴道三维超声对宫内节育器位置异常诊断的价值

目的 研究经阴道三维超声对宫内节育器(IUD)位置异常诊断的价值.方法 选取2020年5月至2021年4月郑州大学附属郑州中心医院疑似IUD位置异常患者98例,均进行经阴道二维超声与经阴道三维超声检查.以宫腔镜检查结果为"金标准",统计经阴道二维超声与经阴道三维超声的诊断结果和诊断效能,分析阳性预测值和阴性预测值,比较经阴道二维超声与经阴道三维超声对不同IUD位置异常类型的检出率.结果 本组98例疑似IUD位置异常患者中,经宫腔镜检查确诊阳性74例,阴性24例;经阴道二维超声诊断出阳性56例,阴性16例;经阴道三维超声诊断出阳性71例,阴性23例;经阴道三维超声诊断灵敏度[95.95%(71/74)]、特异度[95.83%(23/24)]、准确度[95.92%(94/98)]较经阴道二维超声诊断[75.68%(56/74)、66.67%(16/24)、73.47%(72/98)]高,漏诊率[4.05%(3/74)]、误诊率[4.17%(1/24)]较经阴道二维超声诊断[24.32%(18/74)、33.33%(8/24)]低(P<0.05);经阴道三维超声诊断阳性预测值[98.61%(71/72)]、阴性预测值[88.46%(23/26)]较经阴道二维超声诊断[87.50%(56/64)、47.06%(16/34)]高(P<0.05);经阴道三维超声对不同IUD位置异常类型的检出率[95.95%(71/74)]较经阴道二维超声[75.68%(56/74)]高(P<0.05).结论 经阴道三维超声应用于IUD位置异常诊断中,能提升诊断灵敏度、特异度及准确度,降低漏诊、误诊发生率,且该诊断方法能确定IUD位置异常类型,为临床开展针对性治疗提供参考依据.
Value of Transvaginal 3D Ultrasound in Diagnosing Abnormal Position of Intrauterine Device
Objective To investigate the value of transvaginal 3D ultrasound in diagnosing abnormal position of intrauterine device(IUD).Methods A total of 98 suspected cases with abnormal position of IUD in Zhengzhou Central Hospital Affiliated to Zhengzhou University from May 2020 to April 2021 were selected and underwent both transvaginal 2D and 3D ultrasound examinations.With hysteroscopy results as the"gold standard",the diagnostic results and diagnostic efficacy of transvaginal 2D and 3D ultrasound were statistically analyzed.The positive predictive value and negative predictive value were analyzed,and the detection rate of different types of abnormal position of IUD by transvaginal 2D and 3D ultrasound was compared.Results Among the 98 suspected cases with abnormal position of IUD,74 cases were diagnosed positive and 24 were diagnosed negative by hysteroscopy.Transvaginal 2D ultrasound diagnosed 56 positive cases and 16 negative cases,while transvaginal 3D ultrasound diagnosed 71 positive cases and 23 negative cases.The diagnostic sensitivity,specificity,and accuracy of transvaginal 3D ultrasound were 95.95%(71/74),95.83%(23/24),and 95.92%(94/98),respectively,which were higher than those of transvaginal 2D ultrasound[75.68%(56/74),66.67%(16/24),73.47%(72/98)].The missed diagnosis rate[4.05%(3/74)]and misdiagnosis rate[4.17%(1/24)]were lower than those of transvaginal 2D ultrasound diagnosis[24.32%(18/74),33.33%(8/24)](P<0.05).The positive predictive value and negative predictive value of transvaginal 3D ultrasound[98.61%(71/72),88.46%(23/26)]were also higher than those of transvaginal 2D ultrasound[87.50%(56/64),47.06%(16/34)](P<0.05).The detection rate of transvaginal 3D ultrasound for different types of abnormal position of IUD was 95.95%(71/74),which was higher than that of transvaginal 2D ultrasound[75.68%(56/74)](P<0.05).Conclusion Transvaginal 3D ultrasound can improve the diagnostic sensitivity,specificity,and accuracy of abnormal position of IUD,reduce the incidence of missed diagnosis and misdiagnosis,and determine the type of abnormal position,providing a reference for targeted clinical treatment.
