Differential Diagnosis of Thyroid Nodules by Superb Microvascular Imaging Combined with Shear Wave Elastography
Objective To analyze the differential diagnosis of thyroid nodules by superb microvascular imaging(SMI)combined with shear wave elastography(SWE).Methods A total of 87 patients with thyroid nodules in the First People's Hospital of Pingdingshan from March 2018 to November 2022 were selected,all of whom underwent SMI and SWE examination.The pathological results were used as the"gold standard"to statistically compare the diagnostic results,diagnostic efficacy and diagnostic predictive value of the identification of thyroid nodules by SMI and SWE alone and in combination.The SWE parameters[maximum modulus of elasticity(Emax),minimum modulus of elasticity(Emin)and mean modulus of elasticity(Emean)]of malignant and benign nodules were compared.Results In 87 patients with thyroid nodules,101 nodules were detected.Pathological findings confirmed malignant 64 cases and benign 37 cases.Sixty-five cases of malignant(55 true malignant cases,10 false malignant cases)and 36 cases of benign(27 true benign cases,9 false benign cases)were diagnosed by SMI.Sixty-five cases of malignant(54 true malignant cases,11 false malignant cases)and 36 cases of benign(26 true benign cases,10 false benign cases)were diagnosed by SWE.Sixty-four cases of malignancy(62 true malignancy,2 false malignancy)and 37 cases of benign(35 true benign,2 false benign)were diagnosed by SMI combined with SWE.The sensitivity,specificity and accuracy of SMI combined with SWE were higher than those of SMI and SWE alone,and the rate of missed diagnosis and misdiagnosis was lower than those of SMI and SWE alone(P<0.05).The positive prognostic value and negative prognostic value of SMI combined with SWE were higher than those of SMI and SWE alone(P<0.05).The Emax,Emin and Emean parameters of malignant nodules were higher than those of benign nodules(P<0.05).Conclusion SMI combined with SWE in the differential diagnosis of thyroid nodule can accurately evaluate the nodule properties and provide reference for clinical diagnosis and treatment.
