Changes of Glycolipid Metabolism and Its Relationship with Endometrial Receptivity in PCOS Infertility Patients
Objective To explore the changes of glucose and lipid metabolism and its relationship with endometrial receptivity in infertile patients with polycystic ovary syndrome(PCOS).Methods A total of 136 patients with PCOS infertility admitted to Shangqiu Maternal and Child Health Care Hospital from March 2021 to October 2022 were selected as the disease group,and 136 healthy women who underwent physical examination at our hospital's physical examination center and identified infertility due to male factors were selected as the health group.Glucose and lipid metabolism indexes[fasting blood glucose(FBG),fasting insulin(FINS),homeostasis model assessment of insulin resistance index(HOMA-IR),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),triglycerides(TG)],uterine spiral artery pulsation index(PI),resistance index(RI)and endometrial thickness(Em)were measured and compared between the two groups.Pearson method was used to analyze the correlation between changes of glucose and lipid metabolism and PI,RI of uterine spiral arteries and Em in patients with PCOS infertility.In addition,patients in the disease group were divided into good endometrial receptivity group and poor endometrial receptivity group according to endometrial receptivity group,and the changes of glycolipid metabolism in the two groups were compared,and the influence of glycolipid metabolism changes on endometrial receptivity in patients with PCOS infertility was analyzed using multivariate logistic regression.Results The levels of FBG,FINS,TC,TG,LDL-C,HOMA-IR and PI,RI of uterine spiral arteries in the disease group were higher than those in the healthy group(P<0.05),while the levels of HDL-C and Em were lower than those in the healthy group(P<0.05).Pearson analysis showed that FBG,FINS,TC,TG,LDL-C,HOMA-IR in patients with PCOS infertility were positively correlated with PI and RI(P<0.05),and negatively correlated with Em(P<0.05).The level of HDL-C was negatively correlated with PI and RI(P<0.05),and positively correlated with Em(P<0.05).The levels of FBG,FINS,TC,TG,LDL-C,HOMA-IR and PI,RI of uterine spiral arteries in the poor endometrial receptivity group were higher than those in the group with good endometrial receptivity(P<0.05),while the levels of HDL-C and Em were lower than those in the good endometrial receptivity group(P<0.05).Multivariate logistic regression analysis showed that FBG,FINS,HOMA-IR,TC,TG,LDL-C,and HDL-C were all influential factors for poor endometrial receptivity in patients with PCOS infertility(P<0.05).Conclusion The levels of FBG,FINS,TC,TG,LDL-C and HOMA-IR in patients with PCOS infertility are all increased,while the level of HDL-C is decreased,which are all associated with PI,RI of uterine spiral arteries and Em,and the disorder of glucose and lipid metabolism is a risk factor for poor endometrial receptivity in patients with PCOS infertility.
