Related Factors of Biliary Tract Infection and Acute Pancreatitis After Endoscopic Retrograde Cholangio Pancreatography in Elderly Patients with Common Bile Duct Stones
Objective To explore the related factors of biliary tract infection and acute pancreatitis after endoscopic retrograde cholangiopancreatography(ERCP)in elderly patients with common bile duct stones.Methods A retrospective analysis was conducted on the clinical data of 140 elderly patients with common bile duct stones who received ERCP treatment at Zhengzhou Central Hospital Affiliated with Zhengzhou University from July 2021 to December 2022.The therapeutic effect of ERCP and the related factors of postoperative complications were observed.Results Among the 140 elderly patients with ERCP,138 were successfully intubated,with a success rate of 98.57%.A total of 129 patients were successfully removed with a one-time stone removal rate of 93.47%.Seven patients developed biliary tract infections after surgery,14 cases developed acute pancreatitis.Single factor and multi factor logistic regression analysis showed that high biliary obstruction and diabetes were independent risk factors for biliary tract infection after ERCP in elderly patients with common bile duct stones(P<0.05).A history of chronic pancreatitis,multiple entry of the catheter into the pancreatic duct,and difficulty in intubation are independent risk factors for acute pancreatitis after ERCP in elderly patients with common bile duct stones(P<0.05).Conclusion The main influencing factors for postoperative complications of ERCP in elderly patients with choledocholithiasis are high biliary obstruction,diabetes mellitus,previous history of chronic pancreatitis,multiple entry of guide wire into pancreatic duct,and difficult intubation.
