Correlation of Serum Levels of TGF-β1,Smad2,Smad3,HA,LN,PC Ⅲ,CⅣat Admission with the Severity of CHB Liver Fibrosis and the Predictive Value of Combined Detection for Disease Prognosis
Objective To investigate the correlation of serum levels of transforming growth factor-β1(TGF-β1),Smad homologous protein 2(Smad2),Smad homologous protein 3(Smad3),hyaluronic acid(HA),type Ⅲ procollagen(PC Ⅲ),laminin(LN)and type Ⅳ collagen(CⅣ)levels with the severity of chronic hepatitis B(CHB)liver fibrosis and the predictive value of combined detection for disease prognosis.Methods A total of 78 patients with CHB liver fibrosis admitted to Henan Hospital of Traditional Chinese Medicine from March 2021 to March 2022 were selected as the study group,and 78 healthy subjects in the same period were selected as the control group.The serum levels of TGF-β1,Smad2,Smad3,HA,PC Ⅲ,LN and CⅣ at admission were compared between the study group and the control group,and among different stages of liver fibrosis and different levels of inflammatory activity in CHB patients with liver fibrosis.The correlation of serum levels of TGF-β1,Smad2,Smad3,HA,PC Ⅲ,LN,CⅣ at admission with the stage of hepatic fibrosis and the grade of inflammatory activity was analyzed.After 3 months of treatment,according to the different prognosis,the CHB liver fibrosis patients were divided into good prognosis and poor prognosis subgroups.The serum levels of TGF-β1,Smad2,Smad3,HA,PC Ⅲ,LN and CⅣ at admission were compared between good prognosis and poor prognosis of CHB liver fibrosis patients.The predictive value of TGF-β1,Smad2,Smad3,HA,PC Ⅲ,LN and CⅣ levels at admission combined detection for the prognosis of CHB liver fibrosis patients was analyzed.Results The serumlevels of TGF-β1,Smad2,Smad3,HA,LN,PC Ⅲ and CⅣ at admission in the study group were higher than those in the control group(P<0.05).There were significant differences in serum levels of TGF-β1,Smad2,Smad3,HA,LN,PC Ⅲ and CⅣ at admission in different liver fibrosis stage and different inflammatory activity of patients with CHB liver fibrosis:Sl<S2<S3<S4,Gl<G2<G3<G4(P<0.05).Serum levels of TGF-β1,Smad2,Smad3,HA,LN,PC Ⅲ,C Ⅳ at admission were positively correlated with liver fibrosis stage and inflammatory activity grade(P<0.05).After treatment,the serum levels of TGF-β1,Smad2,Smad3,HA,LN,PC Ⅲ and CⅣ at admission in good prognosis of CHB liver fibrosis patients were lower than those in poor prognosis patients(P<0.05).The combination of TGF-β1/Smads signaling pathway indexes and liver fibrosis indexes predicted the poor prognosis of patients with AUC better than the single detection of each indicator.Conclusion The serum levels of TGF-β1,Smad2,Smad3,HA,PC Ⅲ,LN and CⅣ at admission are highly expressed in CHB liver fibrosi spatients,which are closely related to the stage of hepatic fibrosis and the grade of inflammatory activity.The above serum indicators combined detection has higher prognostic value for the prognosis of CHB liver fibrosis patients,which can be used to evaluate the severity and prognosis of CHB hepatic fibrosis patients,and provide reference for formulating targeted treatment measures.
