Effect of Fiberoptic Bronchoscopy Combined with Alveolar Lavage on Severe Pneumonia
Objective To analyze the effect of fiberoptic bronchoscopy(FOB)combined with alveolar lavage in the treatment of severe pneumonia(SP).Methods The clinical data of 103 patients with SP admitted to the Zhumadian Central Hospital from June 2021 to January 2023 were retrospectively analyzed.According to the treatment methods,they were divided into aseptic sputum aspiration group(55 cases)and combined lavage group(48 cases).The clinical efficacy,sputum drainage,mechanical ventilation time,success rate of first weaning,ICU stay,blood gas index,pulmonary function index and complications were observed in the two groups.Results The total effective rate of the combined lavage group was higher than that of the aseptic sputum aspiration group(P<0.05).The sputum drainage volume and the success rate of first weaning in the combined lavage group were higher than those in the aseptic sputum aspiration group,and the mechanical ventilation time and ICU hospitalization time were shorter than those in the aseptic sputum aspiration group(P<0.05).The levels of C-reactive protein(CRP),interleukin-6(IL-6),procalcitonin(PCT)and white blood cells(WBC)in the combined lavage group were lower than those in the aseptic sputum aspiration group(P<0.05).The levels of oxygen partial pressure(PaO2),oxygenation index(PaO2/FiO2)and oxygen saturation(SpO2)in the combined lavage group were higher than those in the aseptic sputum aspiration group,while the level of PaCO2 was lower than that in the aseptic sputum aspiration group.Forced expiratory volume(FEV1),forced vital capacity(FVC)and FEV1/FVC in the combined lavage group were higher than those in the aseptic sputum aspiration group(P<0.05).There was no significant difference in complications between the two groups(P>0.05).Conclusion FOB sputum aspiration combined with alveolar lavage is effective in the treatment of SP,which can reduce the inflammatory reaction of the body,improve the blood gas index,respiratory function and lung function,and is safe.
