Influencing Factors for All-Cause Mortality in Peritoneal Dialysis Patients and Construction of Risk Models
Objective To analyze the influencing factors of all-cause mortality in peritoneal dialysis patients and construct a risk model for all-cause mortality in patients.Methods The clinical data of 355 patients with peritoneal dialysis in Xinxiang Central Hospital(the Fourth Clinical College of Xinxiang Medical College)from January 2019 to January 2021 were retrospectively collected,and they were divided into case group(80 cases)and control group(275 cases)according to whether all-cause death occurred during the follow-up period.The tendency score of nearest neighbor matching method was used(matching condition:gender,peritoneal dialysis time,and primary disease)were selected from the control group and case group at a ratio of 1∶3 to form the original data set,and the original data set was divided into a training set(224 cases)and a validation set(96 cases)at a ratio of 7∶3.Clinical data,prognostic nutritional index(PNI)and protein biochemical indexes of patients in the training set were analyzed,and influencing factors were screened by Cox regression equation.Build the internal validation of the nomogram model row and the external validation in the validation set.Results Among 355 patients with peritoneal dialysis,22.54%died of all causes and 77.46%survived.The age,body mass index(BMI),PNI score,hemoglobin(Hb),albumin(ALB),prealbumin(PA),transferrin(TF),and uric acid(UA)of the two groups in the training set were compared,and the differences were statistically significant(P<0.05).Cox regression showed that age,PNI score,ALB,Hb,UA were the influencing factors of all-cause death in peritoneal dialysis patients(P<0.05).Based on the above factors,the nomogram prediction model was constructed.The nomogram model with PNI and protein biochemical indexes in training set and validation set calibration curve,ROC curve and decision curve had better predictive ability and added clinical benefits.Conclusion The influencing factors for all-cause mortality in peritoneal dialysis patients are age,PNI score,ALB,Hb,UA.The column chart prediction model based on age,PNI score,ALB,Hb,UA has high predictive value and clinical applicability.
