Factors Affecting CD-RISC Score in Patients with Thyroid Cancer After Surgery
Objective To explore the influencing factors of psychological state in patients with thyroid cancer after surgery, and to provide reference for clinical intervention. Methods A total of 106 patients with postoperative thyroid cancer admitted to the First Affiliated Hospital of Zhengzhou University from September 2021 to September 2023 were retrospectively selected, and their psychological status was assessed with the connor-davidson resilience scale(CD-RISC),and the influencing factors of postoperative CD-RISC score of thyroid cancer patients were analyzed by multiple linear regression. The correlation between CD-RISC score and generalized anxiety disorder 7(GAD-7),patient health questionnaire-9(PHQ-9),Pittsburgh sleep quality index(PSQI),family adaptability and cohesion evaluation scale Ⅱ-Chinese version(FACES Ⅱ-CV),self-perceived burden(SPB)and social support rating scale(SSRS)was analyzed. The influence of each scale on mental state was analyzed by path analysis. Results The CD-RISC score of 106 patients with thyroid cancer was(58.64±4.16)points,which was lower than the domestic norm[(65.40±13.90)points](P<0.05).Age,education level,TNM stage, anxiety, depression, sleep quality, family intimacy and adaptability, social support level,self-perceived burden were the important influencing factors of CD-RISC score(P<0.05).CD-RISC scores were negatively correlated with GAD-7,PHQ-9,PSQI and SPB scores(r=-0.803,-0.716,-0.726,-0.695,P<0.001),and positively correlated with FACES Ⅱ-CV and SSRS scores(r=0.775,0.826,P<0.001).Self-perceived burden, family closeness and adaptability had direct and indirect effects on mental state, while social support, sleep quality, depression and anxiety had direct effects on mental state. Conclusion The psychological state of patients with thyroid cancer after surgery is poor, and the main influencing factors are age, education level,TNM stage, anxiety, depression, sleep quality, family closeness and adaptability, social support level, and self-perceived burden. It is necessary to adopt targeted programs to establish positive coping methods to improve their psychological state.
