目的 探讨瑞马唑仑联合罗哌卡因在断指再植术中的应用效果.方法 回顾性选取2021年10月至2023年4月在医院接受断指再植术的159例患者,按麻醉方案分为参照组(79例)、研究组(80例).其中接受罗哌卡因麻醉的纳入参照组,接受瑞马唑仑联合罗哌卡因麻醉的纳入研究组.以术前(T0)、手术开始时(T1)、手术开始后1 h(T2)、术毕时(T3)为时间节点,比较两组血流动力学指标[心率(HR)、平均动脉压(MAP)]水平、镇静效果[改良警觉/镇静量表(MOAA/S)评分、脑电双频指数(BIS)].比较两组术前、术后24 h氧化应激反应指标[血清超氧化物歧化酶(SOD)、丙二醛(MDA)]水平及不良反应发生情况.结果 两组T1～T3时MAP、HR、MOAA/S、BIS值的组间、时间、交互效应有统计学意义(P＜0.05);研究组术后1 d SOD水平高于参照组,MDA水平低于参照组(P＜0.05);研究组不良反应总发生率[5.00％(4/80)]与参照组[7.59％(6/79)]相比,差异无统计学意义(P＞0.05).结论 在断指再植术中应用瑞马唑仑联合罗哌卡因麻醉可有效提高麻醉质量,稳定患者血流动力学水平,减轻氧化应激损伤,且安全性可靠.
Application Effect of Remazolam Combined with Ropivacaine in Replantation of Severed Finger
Objective To investigate the effect of remazolam combined with ropivacaine in replantation of severed finger.Methods A total of 159 patients who underwent severed finger replantation in the hospital from October 2021 to April 2023 were retrospectively selected and divided into the reference group(79 cases)and the study group(80 cases)according to anesthesia protocols.Ropivacaine anesthesia was included in the reference group,and remazolam combined with ropivacaine anesthesia was included in the study group.The hemodynamic indexes[heart rate(HR),mean arterial pressure(MAP)]and sedation effect[modified vigilance/sedation scale(MOAA/S)score,bispectral index(BIS)]of the two groups were compared before surgery(T0),at the beginning of surgery(T,),1 hour after surgery(T2),and at the end of surgery(T3).The levels of oxidative stress index[serum superoxide dismutase(SOD)and malondialdehyde(MDA)]and the occurrence of adverse reactions were compared between the two groups before and 24 hours after operation.Results The intergroup,time and interaction effects of MAP,HR,MOAA/S and BIS value of two groups were statistically significant(P＜0.05).The SOD level of the study group was higher than that of the reference group,and the MDA level was lower than that of the reference group(P＜0.05).The total incidence of adverse reactions in the study group was 5.00％(4/80),which was not statistically significant compared with 7.59％(6/79)in the reference group(P＞0.05).Conclusion Remazolam combined with ropivacaine can effectively improve the quality of anesthesia,stabilize the hemodynamic level of patients,reduce oxidative stress damage,and is safe and reliable.
