Relationship Between Serum Myelin Basic Protein Expression and Bleeding Transformation After Intravenous Thrombolysis with rt-PA in Patients with Acute Cerebral Infarct
Objective To investigate the relationship between the expression of serum myelin basic protein(MBP)and haemorrhagic transformation(HT)after intravenous thrombolysis with recombinant tissue type plasmin activator(rt-PA)in patients with acute cerebral infarct(ACI).Methods A prospective study was conducted on 105 ACI patients admitted to the First People's Hospital of Shangqiu from May 2019 to December 2021.All patients received rt-PA intravenous thrombolysis treatment and were divided into HT group and non HT group based on the occurrence of HT within 24 hours after treatment.The general data,serum MBP levels before treatment and other laboratory indicators were compared between the two groups,and the relationship between serum MBP and HT after rt-PA intravenous thrombolysis in ACI patients was analyzed.Results Among the 105 patients,15 had HT,with an incidence rate of 14.29%.The National Institutes of Health stroke scale(NIHSS)score of the HT group before thrombolytic therapy was higher than that of the non HT group,and the time from onset to treatment was longer than that of the non HT group.The serum MBP level was higher than that of the non HT group,and the difference was statistically significant(P<0.05).Point two column correlation analysis showed that serum MBP was associated with the occurrence of HT in ACI patients after rt-PA intravenous thrombolysis(P<0.05).Multivariate logistic analysis found that NIHSS score before thrombolysis treatment,time from onset to treatment,and serum MBP level were all influencing factors for the occurrence of HT in ACI patients after rt-PA intravenous thrombolysis(P<0.05).The analysis of the receiver operating characteristic(ROC)curve showed that the area under the curve of serum MBP predicting HT occurrence in ACI patients after rt-PA intravenous thrombolysis was 0.740(95%CI:0.606-0.874),which had certain predictive value.Conclusion The serum MBP level is closely related to the occurrence of HT after rt-PA intravenous thrombolysis in ACI patients,and can serve as an auxiliary indicator for early clinical prediction of the risk of H T after rt-PA intravenous thrombolysis in patients.
