Relationship Between DBP Gene Polymorphism and Poor Prognosis of Intravenous Thrombolysis in Acute Cerebral Infarction
Objective To explore the relationship between vitamin D binding protein(DBP)gene polymorphism and poor prognosis of intravenous thrombolysis in acute cerebral infarction(ACI).Methods A total of 177 patients with ACI who were admitted to the Third People's Hospital of Henan Province for intravenous thrombolytic therapy from June 2019 to June 2022 were selected as the disease group,and 165 healthy people who underwent physical examination at the same time were selected as the control group.Real-time fluorescence quantitative PCR was used to detect the DBP gene polymorphism of the two groups.Hardy-Weinberg genetic balance was used to test the genotype distribution of the two groups,and the distribution of genotypes and alleles was compared between the two groups.After 3 months of treatment,the disease group was divided into good prognosis group and poor prognosis group based on the prognosis.The distribution of genotype and allele in the two groups was compared.The factors affecting the poor prognosis of intravenous thrombolysis in ACI patients were analyzed using multivariate logistic regression.Results The frequencies of AA genotype and A allele of DBP-rs4588 locus in the disease group were higher than those in the control group(P<0.05),while the frequencies of CC genotype and C allele were lower than those in the control group(P<0.05).The frequencies of AA genotype and A allele of DBP-rs4588 locus in the poor prognosis group were higher than those in the good prognosis group(P<0.05),while the frequencies of CC genotype and C allele were lower than those in the good prognosis group(P<0.05).The proportion of hypertension,time from onset to treatment,the level of C-reactive protein at admission,the white blood cell count at admission,the neutrophil lymphocyte ratio at admission and the score of National Institute of Health stroke scale(NIHSS)at admission in the poor prognosis group were higher than those in the good prognosis group(P<0.05).The AA genotype and A allele at DBP-rs4588 locus,hypertension,time from onset to treatment,and NIHSS score at admission were all risk factors for poor prognosis in ACI patients after thrombolysis(P<0.05).Conclusion The genotype and allele distribution of DBP-rs4588 locus in patients with different prognosis of ACI are different,and the AA genotype and A allele of DBP-rs4588 locus are all influencing factors for poor prognosis of intravenous thrombolysis in ACI patients.
