Application of Cluster Nursing Interventions Combined with PDCA Cycle in the Management of Venous Thromboembolism After Total Hip Arthroplasty in Elderly Orthopedic Patients
Objective To investigate the application of cluster nursing intervention combined with PDCA cycle in the management of venous thromboembolism(VTE)after orthopedic total hip arthroplasty in elderly patients.Methods The elderly patients after hip arthroplasty were selected from the 988th Hospital of the PLA Joint Logistic Support Force.Patients admitted from June to December 2022 received routine care as the control group,and patients admitted from January to June 2023 received cluster nursing intervention combined with PDCA cycle nursing on the basis of routine care as the observation group,with 60 patients in each group.The self-care ability,incidence of VTE within 1 month after surgery,peak velocity and mean velocity of femoral vein,and patient satisfaction during treatment were compared between the two groups.Results Before nursing,there was no difference in the self-care ability scores of patients in both groups(P>0.05).After nursing,the self-care ability scores and satisfaction of patients in the observation group were higher than those in the control group(P<0.05).The peak velocity and mean velocity of femoral vein were comparable between the two groups before surgery and on the first day after surgery(P>0.05),but there were differences on the 7 th day after surgery(P<0.05).The incidence of VTE within 1 month after surgery was lower in the observation group than that in the control group(P<0.05).Conclusion Implementing cluster nursing intervention combined with PDCA cycle management for patients after total hip arthroplasty can enhance patients'self-care ability and satisfaction,and reduces the incidence of postoperative VTE.
