Significance of Detecting Peripheral Blood Chemerin,Irisin and 25(OH)VD Levels in Patients with Type 2 Diabetes Mellitus Complicated with Sarcopenia
Objective To investigate the changes and detection significance of peripheral blood Chemerin,Irisin and 25-hydroxyvitamin D[25(OH)VD]levels in patients with type 2 diabetes mellitus complicated with sarcopenia(SP).Methods A total of 156 outpatients and inpatients admitted to the Endocrinology Department of Zhengzhou Third People's Hospital from January 2021 to May 2022 were selected, and divided into SP group(simple SP,46 cases),diabetes group(simple type 2 diabetes mellitus,69 cases),and diabetes+SP group(type 2 diabetes mellitus complicated with SP,54 cases).Fifty healthy subjects who received physical examination in the hospital during the same period were selected as the control group. The general clinical data, peripheral blood Chemerin,Irisin and 25(OH)VD levels of different groups were compared.Logistic regression analysis was performed to screen the influencing factors of SP in patients with type 2 diabetes mellitus. The receiver operating characteristic(ROC)curve was used to analyze the predictive value of peripheral blood Chemerin,Irisin and 25(OH)VD levels for SP in type 2 diabetes mellitus. Results Body mass index(BMI),fasting plasma glucose(FPG),glycosylated hemoglobin(HbA1c),fasting insulin(FINS),triacylglycerol(TG)and low density lipoprotein(LDL)levels were significantly different in different groups(P<0.05).Peripheral blood Chemerin levels in the SP group, the diabetes group and the diabetes+SP group were higher than that in the control group, and the levels of Irisin and 25(OH)VD were lower than those in the control group(P<0.05).Peripheral blood Chemerin level in the diabetes+SP group was higher than those in the SP group and the diabetes group, while the levels of Irisin and 25(OH)VD were lower than those in the SP group and the diabetes group(P<0.05).Multivariate logistic regression analysis showed that FPG,HbAlc,TG and Chemerin were independent risk factors for SP(P<0.05),while Irisin and 25(OH)VD were protective factors(P<0.05).ROC curve analysis showed that the AUC values of peripheral blood Chemerin,Irisin and 25(OH)VD levels to predict type 2 diabetes mellitus complicated with SP were 0.712,0.770,and 0.705,respectively(P<0.05).Conclusion Peripheral blood Chemerin level is relatively high, and the levels of Irisin and 25(OH)VD are relatively low in patients with type 2 diabetes mellitus complicated with SP.Detecting the levels of these indicators is helpful for clinical risk assessment of type 2 diabetes mellitus complicated with SP.
