Relationship Between Preoperative Bone Metabolism Level and Occurrence of Femoral Head Necrosis After Internal Fixation in Patients with Femoral Neck Fracture
Objective To explore the relationship between preoperative bone metabolism indicators and the occurrence of femoral head necrosis in patients with femoral neck fractures after internal fixation,in order to provide a basis for optimizing treatment plans.Methods A total of 96 patients with femoral neck fracture admitted to Luoyang Orthopedic-Traumatological Hospital of Henan Province from January 2021 to June 2022 were selected as the study subjects.All patients were treated with internal fixation after definite diagnosis.Questionnaires were designed to collect and compare baseline data of all patients.Bone metabolism indexes[osteocalcin(OST),N-terminal propeptide of type Ⅰ procollagen(PⅠ NP),type Ⅰ collagen carboxyl-terminal peptide β-special sequence(β-CTX),bone specific alkaline phosphatase(BAP)]were recorded on the day of admission.The femoral head necrosis of all patients within 3 months after operation was counted.The relationship between preoperative bone metabolism indexes and femoral head necrosis after internal fixation in patients with femoral neck fracture was analyzed by point two column correlation.Results Within 3 months after operation,17 cases of femoral head necrosis occurred in 96 patients with femoral neck fracture,accounting for 17.71%.The preoperative levels of OST,P Ⅰ NP and BAP in the group with femoral head necrosis were lower than those in the group without femoral head necrosis,and the level of β-CTX was higher than that in the group without femoral head necrosis,the proportion of Garden type Ⅲ was higher than that in the group without femoral head necrosis,with a statistical significant difference(P<0.05).The correlation results of point two column showed that the preoperative levels of OST,P Ⅰ NP,BAP and β-CTX were related to the femoral head necrosis in patients with femoral neck fracture after operation(P<0.05).Conclusion The level of preoperative bone metabolism index is related to the occurrence of femoral head necrosis in patients with femoral neck fracture after operation,and the lower the level of OST,P Ⅰ NP,BAP,the higher the level of β-CTX,the greater the possibility of femoral head necrosis.
