Application Effect of Ultrasound-Guided Femoral Nerve Block Anesthesia in Elderly Hip Fracture Surgery
Objective To analyze the analgesic effect and postoperative cognitive function of ultrasound-guided femoral nerve block anesthesia in elderly hip fracture surgery.Methods A total of 102 elderly patients who underwent hip fracture surgery in Hebi Coal Industry(Group)Co.,Ltd.General Hospital from January to December 2022 were randomly divided into two groups:the control group(51 cases)and the study group(51 cases).The control group received routine spinal anesthesia,while the study group received ultrasound guided femoral nerve block anesthesia.The perioperative related indicators,visual analogue scale(VAS)scores and Ramsay scores at 6,12 and 24 hours after surgery,the score of mini-mental state examination(MMSE)before operation,1 day after operation,2 days after operation,and 3 days after surgery,and occurrence of adverse reactions of the two groups were compared.Results There were no differences between the study group and the control group in terms of the operation time and the amount of intraoperative bleeding(P>0.05).Compared with the control group,the amount of patient-controlled intravenous analgesia(PCIA)used in the study group 24 hours after surgery was less(P<0.05).Compared with the control group,the activity time out of bed and hospital stay in the study group were shorter(P<0.05).At 6,12,and 24 hours after surgery,the VAS of the study group was lower than that of the control group(P<0.05),while the Ramsay score of the study group was higher than that of the control group(P<0.05).On the 1 st,2nd,and 3rd day after surgery,the MMSE scores of both groups decreased compared to before surgery,but the MMSE scores of the study group were higher than those of the control group(P<0.05).The incidence of adverse reactions in the study group was not statistically significant compared to the control group(P>0.05).Conclusion The application of ultrasound guided femoral nerve block anesthesia in elderly hip fracture surgery can effectively reduce the amount of PCIA 24 hours after surgery,shorten the time of getting out of bed and hospitalization,improve the analgesic and sedative effect,reduce postoperative cognitive dysfunction,and has high safety.
