Early Predictive Value of Bedside Index for Severity Acute Pancreatitis and Systemic Immune Inflammation Index for Acute Severe Pancreatitis
Objective To explore the early predictive value of systemic immune inflammation index(SII)combined with bedside index for severity acute pancreatitis(BISAP)in severe acute pancreatitis.Methods Retrospective analysis of clinical data of 206 confirmed acute pancreatitis(AP)patients admitted to Zhengzhou Second Hospital from January 2022 to March 2023.According to the severity of the condition,they were divided into mild acute pancreatitis(MAP)group(111 cases),moderate acute pancreatitis(MSAP)group(43 cases),and severe acute pancreatitis(SAP)group(52 cases).The general information and laboratory test results of each group for 24 hours of admission were recorded,the SII value and BISAP score of the patient on the day of admission were calculated,the differences in general information,laboratory test results,SII value and BISAP score were compared between groups,and the receiver operating characteristic(ROC)curves were draw to analyze the predictive performance of each indicator.Results There was no statistically significant difference in general information among the three groups of patients(P>0.05).In the laboratory indicators,the three groups were compared in platelet count(PLT),neutrophil count(NEUT),lymphocyte count(LYM),blood calcium(Ca2+),blood amylase(AMY),lipase(LIP),SII value,and BISAP score,and the differences were statistically significant(P<0.05).Binary logistic regression analysis showed that Ca2+,AMY,SII value and BISAP score were independent risk factors for SAP patients.The area under the ROC curve of the combination of SII value and BISAP score was 0.964,with sensitivity of 92.3%and specificity of 88.3%(P<0.001),which was superior to the individual evaluation of SII value and BISAP score.Conclusion Ca2+,AMY,SII value and BISAP score are independent risk factors for SAP occurrence.The combination of SII value and BISAP score can serve as a simple,economical,and feasible method for early prediction of SAP.
